Recurrence after fistula surgery?

Hello Dr.
Ke, last August I noticed a bump that looked like a pimple at the anal opening while using the restroom, and there was also purulent material in the toilet.
Therefore, I underwent a fistula surgery at the end of September last year.
It has been almost six months since then, but in the past month, I have noticed purulent material in the toilet two or three times after using the restroom, although I haven't felt any bumps.
Could this indicate a recurrence of the fistula? Thank you, doctor!

Xiao Wen, 30~39 year old female. Ask Date: 2019/03/20

Dr. Ke Fangxu reply Surgery

Based on your description, it seems possible that there may be a recurrence; however, it is essential for a physician to conduct a personal examination to confirm this.
Please have the surgeon evaluate the situation again, and if necessary, you may need to undergo another surgery.

Hello! Thank you for reaching out with your concerns regarding your health after undergoing fistula surgery. It's understandable to feel anxious about potential recurrence, especially after having gone through a surgical procedure. Let's break down your situation and address your concerns.

First, it’s important to understand that the presence of pus-like discharge after a fistula surgery can be concerning, but it does not automatically indicate a recurrence of the fistula. Fistulas are abnormal connections between two body parts, often resulting from infection or inflammation, and they can be complex to treat. After surgery, it is not uncommon for some patients to experience drainage as the surgical site heals. This drainage can sometimes be mistaken for a recurrence, especially if it resembles the discharge you experienced prior to surgery.

In your case, since you mentioned that you have not felt any hard lumps or masses, it may suggest that the surgical site is still healing and that the discharge could be a result of residual inflammation or minor complications rather than a full-blown recurrence of the fistula. However, the fact that you are noticing pus-like material is significant and should not be ignored.

Here are a few steps you can take:
1. Monitor Symptoms: Keep track of any additional symptoms you may experience, such as increased pain, swelling, fever, or changes in the nature of the discharge. If you notice any of these symptoms, it would be wise to seek medical attention promptly.

2. Consult Your Surgeon: It’s crucial to follow up with your surgeon or a colorectal specialist. They can perform a physical examination and possibly imaging studies to assess the surgical site and determine if there is any recurrence of the fistula or if the discharge is due to another issue.

3. Consider Other Causes: Sometimes, drainage can occur due to other reasons, such as a small abscess or a reaction to the surgical site. Your healthcare provider can help differentiate between these possibilities.

4. Follow Post-Operative Care Instructions: Ensure that you are adhering to any post-operative care instructions provided by your surgeon. This may include keeping the area clean, avoiding certain activities, and possibly using sitz baths to promote healing.

5. Be Open About Your Concerns: When you visit your doctor, be open about your worries regarding recurrence. They can provide reassurance and clarify what signs to look for that would indicate a more serious issue.

In summary, while the presence of pus-like discharge can be concerning, it does not definitively mean that your fistula has recurred. It’s essential to consult with your healthcare provider to evaluate your symptoms properly. They will be able to provide you with the most accurate diagnosis and treatment options based on a thorough assessment of your condition.

Remember, post-surgical recovery can take time, and it’s important to be patient with your body as it heals. I hope this information helps alleviate some of your concerns, and I wish you a smooth recovery!
