Postoperative Issues After Hemorrhoid Surgery
Hello Dr.
Ke, I had traditional surgery for internal and external hemorrhoids on February 15th of this year.
I returned for a follow-up appointment a week after the surgery, and the doctor said everything was fine, so I didn't need to come back.
I would like to ask you:
1.
Currently, I sometimes experience fluid discharge from my anus with an unusual odor.
It was better two to three weeks ago, but it has returned recently.
Is this normal two months post-surgery?
2.
There is a small piece of tissue near my anus.
The doctor initially said that the hemorrhoids were completely removed.
Is this excess skin? Additionally, this tissue sometimes appears larger and at other times feels retracted.
Is this due to the ongoing recovery process? How long will it take for this to stabilize?
Thank you for your assistance with these questions.

Dr. Ke Fangxu reply Surgery
1.
It is unusual to still experience this phenomenon two months after surgery; it is best to have the surgeon check it out.
2.
After hemorrhoid surgery, there may be the occurrence of excess skin.
If there is no discomfort, it is generally recommended not to treat it.
You can apply topical ointments; however, if there is severe discomfort, surgical removal may be an option.

After undergoing hemorrhoid surgery, it's common for patients to have concerns regarding their recovery and any unusual symptoms they may experience. Let's address your questions one by one.
1. Fluid Discharge and Odor: Experiencing some fluid discharge from the anal area after surgery can be concerning, especially if it has a foul odor. While some discharge can be normal as the surgical site heals, especially in the first few weeks, persistent or worsening discharge after two months may indicate a potential issue, such as an infection or incomplete healing. It's essential to monitor the characteristics of the discharge—if it is accompanied by increased pain, swelling, or fever, you should seek medical attention promptly. Infections can occur post-surgery, and timely intervention is crucial to prevent complications.
2. Presence of Skin Tag: The small piece of skin you mentioned near the anal area could indeed be a skin tag or residual tissue from the surgery. It’s not uncommon for patients to develop skin tags after hemorrhoid surgery, especially if there was significant tissue manipulation or if the area was previously swollen. The fact that it changes in size could be due to swelling or inflammation as your body continues to heal. Generally, these skin tags are benign and may resolve over time, but they can also persist. If the tag becomes bothersome or if you have concerns about its appearance or any associated symptoms, it would be advisable to consult your surgeon or a colorectal specialist for further evaluation.
Recovery Timeline: Recovery from hemorrhoid surgery can vary significantly from person to person. Typically, most patients can expect to see significant improvement within a few weeks, but complete healing may take several weeks to a few months. During this time, it’s essential to follow post-operative care instructions, including maintaining good hygiene, managing pain, and avoiding straining during bowel movements. A high-fiber diet and adequate hydration can help prevent constipation, which is crucial for a smooth recovery.
When to Seek Help: If you notice any of the following, it’s important to contact your healthcare provider:
- Increased pain or discomfort that does not improve with medication.
- Persistent or worsening discharge, especially if it has a foul odor.
- Signs of infection, such as fever, chills, or increased swelling.
- Any new symptoms that concern you, such as changes in bowel habits or significant changes in the appearance of the surgical site.
In conclusion, while some post-operative symptoms can be normal, it’s essential to remain vigilant and proactive about your recovery. If you have any doubts or if your symptoms persist, don’t hesitate to reach out to your healthcare provider for further guidance. They can provide reassurance, conduct necessary examinations, and ensure that your recovery is on track.
