Post-operative discharge from a fistula?
Hello Doctor: I underwent a fistula and mixed hemorrhoid surgery about four months ago.
My bowel movements have been very smooth, and I mostly have soft stools, so I haven't experienced any difficulties like straining.
However, I notice that almost every time I have a bowel movement in the morning, I have some discharge during the day, but by the time I wake up at night, there is none.
What could this situation indicate?

Answer: Four months after surgery, the wound should be completely healed, and there should be no more wound drainage.
The current phenomenon could be attributed to three possibilities: recurrence of a fistula, postoperative anal incontinence, or excessive intestinal mucus secretion.
To determine the exact cause, a thorough examination by the surgeon is necessary.

More Info
After undergoing fistula surgery and a mixed hemorrhoid procedure, it is not uncommon to experience a variety of symptoms during the recovery phase. Given that you have had a smooth bowel movement process for the past four months, it is a positive sign indicating that your surgical site is healing well. However, the presence of discharge during the day after bowel movements can raise some questions.
The discharge you are experiencing could be due to several factors. First, it is important to consider that after surgery, especially in the anal region, the body may produce mucus as part of the healing process. This mucus can sometimes be mistaken for discharge, especially if it is more noticeable after bowel movements. Additionally, the surgical site may still be sensitive and can produce secretions as it continues to heal.
Another possibility is that the discharge could be related to the fistula itself. If the fistula has not completely healed or if there is still some inflammation in the area, it may lead to intermittent discharge. This is particularly true if the fistula was complex or if there were multiple procedures performed.
It is also worth noting that the presence of discharge can sometimes indicate an infection or inflammation. If the discharge is accompanied by other symptoms such as increased pain, swelling, redness, or fever, it would be advisable to consult your healthcare provider for further evaluation.
In terms of management, maintaining good hygiene is crucial. After each bowel movement, gently clean the area with mild soap and water, and consider using a sitz bath to soothe the area. Keeping the area dry can also help reduce any potential irritation. If the discharge continues or worsens, or if you notice any concerning symptoms, it is essential to follow up with your surgeon or healthcare provider. They may recommend further examination or treatment options based on your specific situation.
In summary, while some discharge after fistula and hemorrhoid surgery can be normal, especially in the early stages of recovery, it is important to monitor the situation closely. If you have any concerns or if the discharge changes in character or frequency, do not hesitate to reach out to your healthcare provider for guidance. Regular follow-ups and open communication with your medical team are key components of a successful recovery.
