Post-ejaculation pain with a strong urge to urinate?
I started masturbating in junior high school, about 1 to 3 times a day.
I have a problem with phimosis (pseudophimosis), and since entering college, my masturbation frequency has decreased to about three times a week.
However, recently after ejaculation, I experience an urgent need to defecate within 2 to 3 minutes, feeling a strong urge to have a bowel movement and urinate, but I am unable to do so.
There is a feeling of fullness in the perineum, and it takes about 3 to 4 hours to relieve myself.
This adverse reaction occurs about once every three times I masturbate.
I am aware that my phimosis may lead to urinary tract infections, so I clean with water daily.
Aside from after ejaculation, I do not experience any pain during urination.

Post-ejaculation pain and urgency can be concerning symptoms, especially when they occur frequently after sexual activity. Based on your description, it seems that you are experiencing a combination of urinary urgency and discomfort in the perineal area following ejaculation. Let's break down the potential causes and considerations regarding your symptoms.
1. Possible Causes: The symptoms you are experiencing could be attributed to several factors. One possibility is that the prolonged pressure or tension in the pelvic region during ejaculation may lead to temporary urinary urgency. This is sometimes referred to as "post-ejaculatory pain syndrome," which can occur in some individuals. Additionally, your history of phimosis (tight foreskin) may contribute to irritation or inflammation of the urethra, which could lead to discomfort and a sensation of urgency after ejaculation.
Another potential cause could be related to pelvic floor muscle tension. If the pelvic floor muscles are overly tense, they can lead to discomfort and a sensation of fullness or urgency. In some cases, this tension can be exacerbated by anxiety or stress related to sexual activity or performance.
2. Impact of Frequency: Reducing the frequency of masturbation may help alleviate your symptoms. Overstimulation of the genital area can lead to temporary discomfort, and allowing your body more time to recover between sessions may help reduce the incidence of post-ejaculatory symptoms. However, it is essential to find a balance that works for you and does not lead to increased anxiety or stress.
3. Infection and Surgical Considerations: If your symptoms are related to an infection, such as a urinary tract infection (UTI) or prostatitis, it may require medical intervention. Infections can lead to inflammation and discomfort, and while many infections can be treated with antibiotics, surgical intervention is typically reserved for more severe cases or structural issues. If you suspect an infection, it is crucial to consult a healthcare provider for appropriate testing and treatment.
4. When to Seek Medical Attention: You should consider seeking medical attention if you experience any of the following:
- Persistent pain or discomfort that does not resolve within a few hours after ejaculation.
- The presence of blood in your urine or semen.
- Fever, chills, or other signs of systemic infection.
- Increased frequency or severity of symptoms.
- Any new or unusual symptoms that concern you.
In conclusion, while your symptoms may be related to the mechanics of ejaculation and pelvic floor tension, it is essential to monitor your condition closely. If the symptoms persist or worsen, it would be wise to consult a urologist or healthcare provider who can perform a thorough evaluation, including a physical examination and possibly some diagnostic tests. They can provide you with a tailored treatment plan based on your specific situation. Remember, maintaining good hygiene and addressing any underlying issues, such as phimosis, may also contribute to improved urinary health.
