Foreign body sensation and swelling after fistula surgery?
Hello doctor, I underwent anal fistula and mixed hemorrhoid surgery along with a colonoscopy on April 1st.
During my follow-up appointment a little over a month post-surgery, the doctor said I had recovered.
However, I still felt pain at that time, so I was prescribed an ointment and suppositories.
Now, more than two months later, I still have a sensation of foreign body and swelling (but there is no discharge).
The suppositories have not provided any improvement.
Is this normal?

Dr. Ke Fangxu reply Surgery
If there is no pain or discharge, it is considered that the anal fistula has healed.
Sometimes, due to a larger wound, the sensation of discomfort may persist for a longer period.
If topical medications are not effective, you might try taking warm sitz baths.

After undergoing surgery for anal fistula and hemorrhoids, it's not uncommon to experience lingering sensations and discomfort, including a feeling of a foreign body and swelling. These symptoms can be concerning, especially if they persist beyond the expected recovery period.
In your case, having had surgery on April 1st and still experiencing these sensations two months later suggests that there may be several factors at play. First, it's important to understand that surgical recovery can vary significantly from person to person. While some individuals may heal quickly, others may experience prolonged symptoms due to various reasons, including the extent of the surgery, individual healing responses, and any underlying conditions.
The sensation of a foreign body can often be attributed to several factors post-surgery. One possibility is that the surgical site is still healing, and the surrounding tissues may be inflamed or sensitive. This inflammation can create a sensation that feels like something is still present in the area. Additionally, if there was significant tissue manipulation during the surgery, it could lead to scarring or adhesions, which may also contribute to these sensations.
Swelling is another common post-operative symptom. It can occur due to inflammation as the body responds to the surgical trauma. While some swelling is expected, persistent or increasing swelling could indicate complications such as infection or fluid accumulation (seroma). It's crucial to monitor the swelling and any changes in symptoms closely.
The fact that you have not noticed any discharge is a positive sign, as it may suggest that there is no active infection. However, the absence of improvement with topical medications and suppositories is concerning. It may indicate that the current treatment plan is not adequately addressing your symptoms.
Given your ongoing discomfort and the lack of improvement, it is advisable to follow up with your surgeon or a healthcare professional specializing in colorectal surgery. They can perform a thorough examination to assess the surgical site, evaluate for any complications, and determine if further intervention is necessary. This may include imaging studies to rule out issues such as abscess formation or other complications that could be contributing to your symptoms.
In summary, while some lingering sensations and swelling can be normal after surgery, the persistence of these symptoms for over two months warrants further evaluation. It's essential to communicate openly with your healthcare provider about your concerns and seek their guidance on the next steps for your recovery. They may recommend additional treatments, adjustments to your current regimen, or further diagnostic evaluations to ensure that your healing process is on track.
