Cauliflower recurrence

Hello Doctor, thank you for taking the time to explain things to me.
I would like to ask about the medication you prescribed for me, which is phenol.
After my last follow-up appointment, I noticed that the lesions have increased in number.
During today’s visit, the doctor asked if I had been applying the medication, and I confirmed that I had.
I asked the doctor if using the medication could cause the lesions to multiply, and the doctor said there is no direct correlation.
However, I found online that some people say the lesions tend to increase after treatment, and I am unsure if the medication is contributing to this.

This time, I am scheduled for a second electrosurgery; I had my first one just a few weeks ago.
Will I need to continue electrosurgery until the lesions are completely gone? The procedure has been quite painful, especially when using the restroom afterward.

Lastly, does smoking and staying up late affect the rapid growth of warts? I read online that boosting the immune system can help; could you please advise on how to enhance my immune system? Thank you.

Dr. Ke Fangxu reply Surgery

I'm not sure if applying phenol can cause an increase, but that seems unlikely.
You might want to consult a dermatologist.
If topical treatments are ineffective, then electrosurgery may be the only option.
Smoking and staying up late shouldn't affect the growth of warts.
Regarding the issue of boosting immunity, that's not my area of expertise, so I recommend consulting a specialist in immunology or rheumatology.

Warts, particularly those caused by the human papillomavirus (HPV), can be a frustrating condition to manage. Your experience with the increasing number of warts despite treatment with topical agents like podophyllotoxin (commonly known as Condylox or Podofilox) is not uncommon. It's important to understand that while these treatments can be effective, they may not eliminate the virus entirely, and warts can recur or multiply due to various factors.

Understanding Wart Recurrence
1. Virus Persistence: HPV can remain dormant in the skin even after visible warts are treated. This means that while the topical treatment may reduce the number of warts, it does not necessarily eradicate the virus from your system. New warts can develop as the virus remains in the skin.

2. Treatment Limitations: Topical treatments like podophyllotoxin are effective for many people, but they may not work for everyone. Some individuals may require more aggressive treatments, such as cryotherapy (freezing), laser therapy, or electrosurgery, especially if they have multiple or stubborn warts.

3. Immune Response: The body’s immune system plays a crucial role in controlling HPV. A strong immune response can help keep the virus in check and reduce the likelihood of new warts forming. Factors that weaken the immune system, such as stress, poor nutrition, lack of sleep, and smoking, can contribute to the recurrence of warts.

Treatment Options
- Electrosurgery: As you mentioned, you have undergone electrosurgery, which is a common method for removing warts. This procedure can be effective, but multiple sessions may be necessary, especially for extensive or recurrent warts. The pain you experience post-treatment is also a common side effect, but it usually subsides with time.

- Combination Therapy: Sometimes, combining treatments can yield better results. For instance, using topical treatments alongside procedures like cryotherapy or electrosurgery may help in managing warts more effectively.

Lifestyle Factors
1. Smoking: There is evidence to suggest that smoking can impair immune function, making it harder for your body to fight off infections, including HPV. Quitting smoking can improve your overall health and potentially enhance your immune response.

2. Sleep and Stress: Lack of sleep and high stress levels can weaken your immune system. Aim for 7-9 hours of quality sleep per night and incorporate stress-reducing activities like exercise, meditation, or hobbies into your routine.

3. Nutrition: A balanced diet rich in vitamins and minerals can support immune health. Foods high in antioxidants (like fruits and vegetables), omega-3 fatty acids (found in fish), and probiotics (found in yogurt) can be beneficial.

Boosting Immunity
To enhance your immune system, consider the following:
- Regular Exercise: Engaging in regular physical activity can improve immune function and overall health.

- Hydration: Staying well-hydrated is essential for maintaining good health and supporting immune function.

- Vitamins and Supplements: Some studies suggest that vitamins C and D, as well as zinc, can support immune health. However, it’s best to consult with a healthcare provider before starting any new supplements.

Conclusion
In summary, while topical treatments like podophyllotoxin are valuable in managing warts, they may not be sufficient for everyone, and recurrence can occur due to the persistence of HPV. Electrosurgery is an effective option, but multiple treatments may be necessary. Lifestyle factors, including smoking, sleep, and nutrition, significantly impact immune function and can influence the recurrence of warts. It’s essential to maintain open communication with your healthcare provider about your treatment options and any concerns you have regarding your condition.
