Unexplained bruising on the foot, recurring?

I have had a wound on my foot since a cut at the swimming pool a year ago, which has formed a scab that has not fallen off for a long time.
I have started to experience unexplained bruising and swelling, which has been increasing.
It is painful when pressed, and the joint where my toes meet the sole of my foot is particularly painful, even though there are no visible scars, leading to insomnia.
I have already seen a dermatologist, but no cause was found.
Blood and urine tests showed no autoimmune issues, and the ultrasound also yielded no results.
After six months, the symptoms resolved, but the dark area (which I am unsure if it is a scar) remained.
Now, it has recurred.
What else can I do?

However, based on the current description, a definitive diagnosis cannot be established.
It is recommended to visit the internal medicine outpatient department at a hospital.
If there are frequent unexplained bruises, a blood test to check for coagulation function issues may be warranted.
For skin scars that do not heal over time, a consultation with a dermatologist may be necessary to determine if there is hypertrophic scarring or keloid predisposition.

Unexplained bruises and swelling, especially in the context of a previous injury, can be concerning and may indicate underlying issues that need to be addressed. Given your history of a swimming pool injury that led to prolonged scabbing and subsequent symptoms, it’s essential to consider several potential causes and avenues for further investigation.

Firstly, the presence of unexplained bruising and swelling could suggest a few different conditions. One possibility is that you may be experiencing a form of chronic inflammation or a localized reaction to the previous injury. Sometimes, the body can react to an old injury with ongoing inflammation, leading to symptoms like swelling and tenderness. This could be exacerbated by activities that put stress on the affected area, such as walking or standing for extended periods.

Another consideration is the possibility of a vascular issue. Conditions such as chronic venous insufficiency can lead to swelling and bruising, particularly in the lower extremities. This occurs when the veins struggle to send blood back to the heart, causing blood to pool in the legs. If you notice that the swelling worsens after prolonged periods of standing or sitting, this could be a factor.

Additionally, while you mentioned that autoimmune conditions were ruled out, it might be worth considering other hematological issues. Conditions like thrombocytopenia (low platelet count) or clotting disorders can lead to easy bruising and prolonged bleeding. Even if initial blood tests did not reveal any issues, it may be beneficial to consult a hematologist for a more comprehensive evaluation, including specific tests for clotting factors.

Given that you have already seen a dermatologist and undergone ultrasound without conclusive results, it may be time to explore other diagnostic avenues. Here are some suggestions:
1. Consult a Vascular Specialist: A vascular surgeon or specialist can evaluate your blood flow and venous health. They may perform tests such as a Doppler ultrasound to assess blood flow in your legs and identify any venous insufficiency.

2. Hematological Evaluation: As mentioned, a hematologist can perform more specialized tests to rule out any blood disorders that could be contributing to your symptoms.

3. Physical Therapy: Engaging in physical therapy may help strengthen the muscles around the affected joints and improve circulation, potentially alleviating some of the swelling and pain.

4. Pain Management: If the pain is significant and affecting your quality of life, discussing pain management options with your primary care physician or a pain specialist may provide relief.

5. Lifestyle Modifications: Consider elevating your legs when resting, wearing compression stockings, and avoiding prolonged periods of inactivity to help manage swelling.

6. Follow-Up Imaging: If symptoms persist, repeat imaging studies, such as an MRI, may provide additional insights into any underlying soft tissue or joint issues that were not visible on previous ultrasounds.

7. Dermatological Follow-Up: If the discoloration persists, a follow-up with a dermatologist specializing in vascular or pigmentary disorders may be warranted to assess the nature of the dark areas left from previous bruising.

In conclusion, while your symptoms have resolved in the past, the recurrence indicates that further investigation is warranted. It’s crucial to communicate your ongoing symptoms clearly to your healthcare providers and advocate for further testing if necessary. Keeping a detailed record of your symptoms, including when they occur and any potential triggers, can also be helpful in guiding your healthcare team toward a diagnosis. Remember, persistent symptoms deserve thorough evaluation, and you should not hesitate to seek a second opinion if you feel your concerns are not being adequately addressed.
