It sounds like a problem related to the sebaceous glands.
It should be benign, but if you have concerns, it is advisable to see a urologist for an examination and discuss it with the physician.

When observing white spots on the foreskin, it is important to understand the potential causes and whether they indicate a medical concern. The appearance of white spots can be attributed to several benign conditions, but it is essential to differentiate these from more serious issues.
One common cause of white spots on the foreskin is the presence of pearly penile papules. These are small, dome-shaped bumps that are typically white or flesh-colored and are considered a normal anatomical variant. They are not harmful and do not require treatment. Pearly penile papules are often mistaken for a sexually transmitted infection (STI) or other pathological conditions, but they are completely benign.
Another possibility is the presence of Fordyce spots, which are small, painless, pale bumps that can appear on the shaft of the penis or the foreskin. These spots are sebaceous glands that are not associated with hair follicles and are also a normal variant. They are harmless and do not require any treatment.
In some cases, white spots can be indicative of a condition known as balanitis, which is inflammation of the glans (the head of the penis) and can sometimes extend to the foreskin. Balanitis can be caused by poor hygiene, infections (such as yeast infections), or irritants. Symptoms may include redness, swelling, and discomfort, but in your case, since there is no itching or pain, this may be less likely.
Another condition to consider is lichen sclerosus, which can cause white patches on the skin, including the genital area. This condition is more common in men who are uncircumcised and can lead to scarring and narrowing of the foreskin if left untreated. However, lichen sclerosus is usually accompanied by other symptoms, such as itching or pain.
If the white spots are not accompanied by any discomfort, itching, or changes in urination, they are likely benign. However, it is always prudent to consult a healthcare professional for a proper evaluation, especially if there are any changes in the appearance of the spots, or if new symptoms develop.
In summary, while white spots on the foreskin can be concerning, many cases are due to harmless conditions such as pearly penile papules or Fordyce spots. If you are uncertain or if the spots change in appearance or are accompanied by other symptoms, it is advisable to seek medical advice for a thorough examination and peace of mind. Regular self-examinations and maintaining good hygiene can also help in monitoring any changes in the genital area.
