Pain during ejaculation?

Hello, Doctor: I would like to ask why I experienced noticeable pain and pulling sensations in my groin during ejaculation while masturbating.
Although the distance of ejaculation was significantly farther than usual, I have never felt this kind of pain during ejaculation before.
Earlier today, I also worked out my legs at the gym; could this be related? After some time, the pain has subsided, and I am urinating normally, but I still feel a bit uneasy.
Could you please provide some insight?

LK, 10~19 year old female. Ask Date: 2019/10/13

Dr. Du Shixi reply Urology

Hello: It may still be related to exercise, so please observe for a few more days.
Persistent pain is considered abnormal.
Do not overtrain.
Wishing you good health.

Experiencing pain during ejaculation can be concerning, and it's understandable that you're feeling anxious about it. There are several potential causes for this discomfort, and it's important to consider both physical and psychological factors.

Firstly, the pain you experienced during ejaculation could be related to muscle strain or tension, especially if you recently engaged in leg workouts at the gym. Intense physical activity can lead to muscle soreness or tightness, which may manifest as discomfort in the pelvic region during sexual arousal or ejaculation. The pelvic floor muscles, which support the bladder and bowel and play a role in sexual function, can become tense or strained from physical exertion. This tension can lead to sensations of pain or pulling during ejaculation.

Another possibility is that the pain could be related to the prostate gland, which is located just below the bladder and surrounds the urethra. Conditions such as prostatitis (inflammation of the prostate) or pelvic floor dysfunction can cause pain during ejaculation. Prostatitis can be acute or chronic and may present with symptoms such as pain in the lower abdomen, pelvic pain, or discomfort during urination or ejaculation. If the pain persists or worsens, it would be wise to consult a healthcare professional for a thorough evaluation.

Additionally, anxiety and stress can play a significant role in physical sensations and pain perception. If you are feeling anxious about your health or bodily functions, this could heighten your awareness of discomfort, making it feel more pronounced. Anxiety can lead to muscle tension throughout the body, including the pelvic area, which may contribute to the pain you experienced.

It's also worth noting that variations in ejaculation, such as distance or force, can occur due to various factors, including arousal levels, physical condition, and even hydration status. While a longer distance of ejaculation is generally not a cause for concern, if it is accompanied by pain, it may warrant further investigation.

In conclusion, while occasional pain during ejaculation can occur and may be linked to muscle strain or tension, it is essential to monitor your symptoms. If the pain recurs, intensifies, or is accompanied by other symptoms such as changes in urination, blood in urine or semen, or persistent discomfort, it is crucial to seek medical advice. A healthcare provider can perform a physical examination, possibly including a prostate exam, and may recommend further tests to rule out any underlying conditions.
In the meantime, consider practicing relaxation techniques to help alleviate anxiety and muscle tension. Gentle stretching, pelvic floor exercises, or mindfulness practices may also be beneficial. Remember, your health and well-being are important, and addressing any concerns with a medical professional can provide peace of mind and appropriate care.
