Severe swelling after removing the bandage on the sixth day?

Hello Doctor: I had a circumcision surgery in Banqiao.
The pain was manageable for the first few days, but on the sixth day, after the bandage was removed, I started experiencing severe swelling.
The swelling is painful to the touch and has made movement very difficult.
Now, on about the ninth day, I still haven't seen any reduction in the swelling (the key issue is that it hurts when touched).
Activities like sleeping, sitting, and walking are quite inconvenient.
Is this normal? Are there any medications that can help me adapt quickly? I am very worried because I need to go to China for work in two weeks.
Thank you, Doctor!

First, position the penis upwards, for example, by wearing briefs or using 3M tape with gauze to secure it in place.
Then, promptly visit a urologist for an examination to check for any infections or other issues, and discuss with the physician how to proceed with treatment.

After undergoing a circumcision procedure, experiencing some swelling is not uncommon, especially in the first week following the surgery. However, severe swelling that persists beyond the initial postoperative period, particularly if it is accompanied by pain and discomfort, warrants further evaluation.

In your case, it seems that you are experiencing significant swelling around the surgical site on the ninth day post-operation. While some degree of swelling can be expected due to the surgical trauma, severe swelling that causes pain and limits mobility may indicate a few potential issues, including:
1. Normal Postoperative Swelling: After any surgical procedure, the body responds with inflammation, which can lead to swelling. This is a natural part of the healing process. However, this swelling should gradually decrease over time.

2. Infection: One of the more concerning possibilities is that the swelling could be due to an infection at the surgical site. Signs of infection may include increased redness, warmth, discharge, or a fever. If you notice any of these symptoms, it is crucial to seek medical attention promptly.

3. Fluid Accumulation (Seroma or Hematoma): Sometimes, fluid can accumulate in the area where the surgery was performed, leading to a seroma (a collection of clear fluid) or hematoma (a collection of blood). These can cause significant swelling and discomfort.

4. Allergic Reaction: If you have been prescribed any medications or topical treatments post-surgery, an allergic reaction could also lead to swelling.

To manage the swelling and discomfort you are experiencing, consider the following steps:
- Rest and Elevation: Try to keep the area elevated as much as possible to reduce swelling. Avoid strenuous activities that could exacerbate the swelling.

- Cold Compress: Applying a cold compress to the swollen area can help reduce inflammation and numb the pain. Make sure to wrap the ice pack in a cloth to avoid direct contact with the skin.

- Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and reduce inflammation. However, consult your doctor before taking any medication, especially if you have any underlying health conditions or are taking other medications.

- Follow-Up with Your Surgeon: Given that you are experiencing significant swelling and discomfort, it is advisable to follow up with the surgeon who performed your circumcision. They can assess the surgical site for any signs of complications and provide appropriate treatment.

- Avoid Irritants: Ensure that the area remains clean and dry. Avoid any irritants, including tight clothing or activities that could cause friction against the surgical site.

Since you mentioned that you have a work commitment in China in two weeks, it is essential to address this issue promptly. If the swelling does not improve or worsens, or if you develop any additional symptoms, seek medical attention immediately. Your healthcare provider may need to drain any fluid accumulation or prescribe antibiotics if an infection is suspected.

In summary, while some swelling is normal after circumcision, severe and persistent swelling accompanied by pain should not be ignored. It is crucial to consult with your healthcare provider to ensure proper healing and to rule out any complications.
