Frequent chest pain?

Hello Dr.
Yang, I used to experience occasional left chest pain, but recently it has been happening frequently over the past few weeks.
Sometimes the pain is intense, while other times it feels like a dull, tight sensation.
The location of the pain is not always the same and is quite vague.
Today, while sitting, the pain started unexpectedly; it began as a mild ache and gradually intensified, eventually radiating to the left side of my back near the shoulder blade.
I also feel tightness in my throat and neck, which lasts for over half an hour.
At its worst, I experience dizziness and a sensation of breathlessness, and I have even fainted suddenly before.
I went to the emergency room, where the doctor performed an X-ray and an electrocardiogram, stating that everything appeared normal except for a slightly elevated heart rate.
They prescribed muscle relaxants and a sedative (Anxin) for me.
This has been happening more frequently, and the doctor mentioned it might be related to stress.
Should I see a specialist for further evaluation? I am concerned about the possibility of a heart issue and whether there is a risk of a myocardial infarction.

Dr. Yang Wenda reply Pulmonology

Hello: The likelihood of a myocardial infarction should be low, but there is concern about paroxysmal arrhythmia.
It is recommended to see a cardiologist and also to learn stress relief techniques!

Frequent chest pain can be a concerning symptom, and it's understandable that you're feeling anxious about it, especially given the episodes of pain you've described. Based on your account, it seems that your chest pain is accompanied by other symptoms such as tightness in the throat and neck, back pain, and even episodes of dizziness and fainting. While your recent evaluations, including X-rays and an electrocardiogram (ECG), did not reveal any immediate cardiac issues, it’s important to consider a comprehensive approach to understanding and managing your symptoms.

Firstly, it’s crucial to recognize that chest pain can arise from various causes, some of which may be benign while others could be more serious. The fact that your pain is intermittent and associated with stress suggests that it could be related to musculoskeletal issues or anxiety. Conditions like costochondritis, which is inflammation of the cartilage connecting the ribs to the sternum, can cause localized chest pain that may mimic heart-related pain. Similarly, anxiety and panic attacks can lead to chest tightness, palpitations, and even feelings of dizziness or faintness.

Given that your symptoms have been increasing in frequency, it would be wise to follow up with a healthcare provider who can perform a thorough evaluation. A visit to a cardiologist may be warranted to rule out any underlying heart conditions, especially if you have risk factors such as a family history of heart disease, high blood pressure, or high cholesterol. They may recommend further testing, such as a stress test or echocardiogram, to assess your heart's function more comprehensively.

Additionally, considering the possibility of anxiety or stress-related disorders is important. If your healthcare provider suspects that your symptoms are linked to anxiety, they might refer you to a mental health professional for further evaluation and management. Cognitive-behavioral therapy (CBT) and relaxation techniques can be effective in managing anxiety and its physical manifestations.

In the meantime, there are several strategies you can adopt to help manage your symptoms. Regular physical activity can improve cardiovascular health and reduce stress levels. Mindfulness practices, such as meditation or yoga, can also be beneficial in managing anxiety and promoting relaxation. Furthermore, ensuring you have a good support system and discussing your feelings with friends or family can alleviate some of the emotional burdens you may be experiencing.

Lastly, keep a symptom diary to track your episodes of chest pain, noting the time of day, duration, and any associated activities or stressors. This information can be invaluable for your healthcare provider in determining the underlying cause of your symptoms.

In summary, while your initial tests have not indicated a serious cardiac issue, it is essential to continue monitoring your symptoms and seek further evaluation as needed. A multidisciplinary approach involving both cardiology and mental health may provide the best pathway to understanding and alleviating your chest pain. Always prioritize your health and do not hesitate to seek immediate medical attention if your symptoms worsen or if you experience new symptoms such as severe shortness of breath, radiating pain, or prolonged dizziness.
