Hydrolyzed Formula: When Is It Necessary for Infants? - Pediatrics

Infant formula

Hello, Doctor: My husband has nasal allergies and atopic dermatitis, and I have a tendency for seasonal urticaria.
Therefore, for our first child, we used hydrolyzed formula until he was under one year old, and after that, we switched to regular formula (both accompanied by probiotics).
We initially planned to do the same for our second child, but I heard from a pharmacist that hydrolyzed formula is only necessary for infants with protein allergies, and that children without protein allergies can drink regular formula.
Is this true? Thank you for your time in responding.

A, 30~39 year old female. Ask Date: 2020/03/02

Dr. Pei Rensheng reply Pediatrics

Hello! Current research shows that hydrolyzed protein formula can prevent allergies in infants, and its preventive effects are not limited to just protein allergies, but extend to general allergies as well.
Wishing you health, happiness, and peace! Sincerely, Rensheng.

Hydrolyzed formula is a specialized type of infant formula that has been broken down into smaller protein fragments, making it easier for infants to digest. It is particularly beneficial for infants who have a higher risk of developing allergies or those who have already shown signs of food allergies, especially to cow's milk protein. Given your family's history of allergies, including your husband's nasal allergies and atopic dermatitis, it's understandable that you are considering the best nutritional options for your second child.

The primary indication for using hydrolyzed formula is indeed for infants who have been diagnosed with cow's milk protein allergy (CMPA) or those who are at high risk of developing allergies due to family history. If your first child had no issues with regular formula after the age of one, it suggests that they may not have had a sensitivity to cow's milk protein. In such cases, transitioning to a standard formula after one year is generally considered safe and appropriate.

However, it is essential to note that not all infants with a family history of allergies will develop them. The decision to use hydrolyzed formula should be based on individual circumstances, including any signs of intolerance or allergy in the infant. If your second child shows any signs of allergic reactions, such as rashes, gastrointestinal distress, or respiratory issues after consuming regular formula, it would be prudent to consult with a pediatrician or an allergist. They may recommend continuing with hydrolyzed formula or conducting allergy testing to determine the best course of action.

In terms of nutritional adequacy, hydrolyzed formulas are designed to meet the nutritional needs of infants just like standard formulas. They contain essential nutrients, including proteins, fats, carbohydrates, vitamins, and minerals. The hydrolyzation process does not compromise the nutritional quality of the formula; rather, it makes it more suitable for infants with specific dietary needs.

When considering whether to use hydrolyzed formula for your second child, here are some key points to keep in mind:
1. Family History of Allergies: Since both you and your husband have allergic conditions, your second child may be at a higher risk for developing allergies. Hydrolyzed formula can be a preventive measure.

2. Signs of Allergy: Monitor your infant for any signs of food allergies, such as skin rashes, vomiting, diarrhea, or respiratory issues after feeding. If any of these symptoms occur, consult a healthcare professional.

3. Consultation with Pediatrician: Before making any changes to your infant's diet, it is always best to discuss with your pediatrician. They can provide personalized recommendations based on your child's health history and family background.

4. Transitioning to Regular Formula: If your second child does not exhibit any signs of allergies and is thriving on hydrolyzed formula, transitioning to a regular formula after one year may be appropriate, similar to your first child.

5. Nutritional Balance: Ensure that any formula you choose provides a balanced diet for your infant, meeting their growth and developmental needs.

In conclusion, while hydrolyzed formula is primarily indicated for infants with documented allergies to cow's milk protein, it can also be a preventive measure for those at high risk due to family history. Always consult with your pediatrician to determine the best feeding strategy for your child, ensuring that their nutritional needs are met while considering any potential allergy risks.
