Post-operative issues related to circumcision and hernia repair?
Hello Dr.
Du,
I underwent a circumcision and inguinal hernia repair surgery at the beginning of February, and today marks the fifth week post-operation.
Around the fourth week, I noticed some dark, slightly raised spots at the site where the stitches were (only on the left, right, and bottom; none appeared on the top).
Each spot is about the same size and evenly spaced, and they are not painful.
Could you please tell me if this is what is referred to as a scar?
I also have a question regarding cleanliness: is it sufficient to clean the glans? Can the foreskin still be retracted for cleaning in the future? My hernia was on the right side of the testicle, and after the surgery, I feel that the right testicle has become more sensitive.
For example, when I squat and then stand up quickly, if my thigh bumps into the testicle, I experience a slight pain in the right testicle.
This pain has recently decreased from mild pain to slight discomfort, but I would like to ask if this is normal.
Regarding hernia recovery, I have read many articles, and the vast majority state that vigorous exercise should be avoided for two months.
If I occasionally experience sharp pain at the hernia repair site, is that acceptable? Even at five weeks post-surgery, I still occasionally feel sharp pain or discomfort.
I saw a Q&A from a certain hospital that mentioned nerve repair at the surgical site can take anywhere from a few weeks to several months.
If, unfortunately, I still experience occasional sharp pain after two months, can I still start exercising (like playing baseball or basketball)?
Thank you for taking the time to help!

Hello: 1) If the color is darker, it may be scar tissue.
2) The foreskin should be retracted for cleaning; remember to dry it thoroughly, avoiding any moisture.
3) After surgery, discomfort is expected, but it will gradually improve; some individuals may take several months.
If there is no recurrence of the hernia, further surgery is not necessary.
However, during the recovery period after surgery, it is essential to follow care instructions to prevent recurrence in the future.
If there is a solid two months of proper rest, exercise can be gradually increased starting with light activities.
Wishing you good health.

Post-operative concerns following circumcision and hernia surgery can be quite common, and it’s important to address them with the right medical insights. Based on your description, let’s break down your concerns into manageable parts.
1. Appearance of Dark Spots: The dark, slightly raised spots you’ve noticed around the circumcision site are likely to be a form of scarring or possibly hypertrophic scars. These types of scars can develop as part of the healing process, especially after surgical procedures. They are generally benign and may fade over time. However, if they become painful, change in color, or increase in size, it would be advisable to consult your surgeon for further evaluation.
2. Cleaning the Area: Post-circumcision care is crucial for preventing infections. You should clean the glans (the head of the penis) gently with warm water. It’s important to avoid using harsh soaps or chemicals that can irritate the sensitive skin. As for retracting the remaining skin (if any), it’s best to follow your surgeon’s advice. In most cases, the skin should not be forcibly retracted until it is fully healed, as this can cause discomfort or complications.
3. Sensitivity of the Right Testicle: The increased sensitivity in your right testicle after hernia surgery is not uncommon. The surgical procedure can lead to temporary nerve irritation or inflammation in the area, which might explain the discomfort you feel when moving suddenly. This sensitivity should gradually improve over time. If the pain persists or worsens, it’s essential to reach out to your healthcare provider for a thorough examination.
4. Post-Hernia Surgery Pain: Experiencing occasional sharp pains or discomfort at the surgical site five weeks post-operation can be part of the normal healing process. Nerve endings in the area may still be recovering, and some patients report sensations of pain or discomfort for weeks or even months after surgery. However, if the pain is severe or accompanied by other symptoms such as swelling, redness, or fever, you should seek medical attention.
5. Resuming Physical Activity: The general guideline for resuming vigorous physical activities, such as playing sports, is typically around six to eight weeks post-surgery. However, this can vary based on individual healing rates and the specifics of your surgery. If you are still experiencing discomfort or pain at the surgical site, it is advisable to hold off on high-impact activities until you have been cleared by your surgeon. Engaging in strenuous activities too soon can lead to complications, including the risk of hernia recurrence.
In summary, while some discomfort and changes in sensation can be normal after circumcision and hernia surgery, it’s crucial to monitor your symptoms closely. If you have any concerns about the healing process, the appearance of your surgical site, or ongoing pain, do not hesitate to contact your healthcare provider. They can provide personalized advice and ensure that your recovery is on track. Always prioritize your health and well-being, and follow up with your surgeon for any persistent or concerning symptoms.
