Post-operative concerns about hemorrhoids?
Hello Doctor:
1.
Yesterday I underwent a hemorrhoidectomy, and the local anesthesia caused me significant pain.
Is it possible to request general anesthesia for this type of surgery instead of local anesthesia?
2.
After the surgery yesterday, I felt very uncomfortable and needed to sleep on my stomach or side.
If I lie flat, will it put pressure on the area?
3.
Today I went to the clinic to change the dressing, but since the nurse told me yesterday that I hadn't had a bowel movement, I didn't remove the outer gauze as instructed.
However, before my follow-up appointment today, I wet the outer gauze while urinating, so I removed it before going to the clinic.
The gauze at the anal area had dried and stuck to the wound.
The nurse advised me to soak it at home, and it would come off naturally, but after soaking, it still hasn't come off.
What should I do?
4.
Regarding the previous question, since the gauze on the wound hasn't come off, can I apply ointment? Would it be effective?
5.
During today's follow-up, the doctor mentioned that they may have only addressed the internal hemorrhoids and that my anal tags might have been obstructed, which is why they may not have been treated.
This really makes me want to be angry.
I went to the doctor thinking that the hemorrhoids would be removed, as the doctor said the anal tags were large and needed surgery.
Now, after the surgery, I'm being told that the tags may not have been treated? Can I still trust this doctor?

Post-surgery recovery from hemorrhoid surgery can be a challenging experience, and it is understandable that you have several concerns following your procedure. Let's address your questions one by one, while also providing some general insights into the recovery process after hemorrhoid surgery.
1. Anesthesia Options: It is common for patients to experience discomfort with local anesthesia, especially in sensitive areas like the anal region. If you found the local anesthesia to be particularly painful, it is reasonable to discuss with your surgeon the possibility of using general anesthesia for future procedures. General anesthesia can provide a more comfortable experience, but it also comes with its own risks and considerations. It is essential to have an open conversation with your healthcare provider about your preferences and any concerns you may have regarding anesthesia.
2. Sleeping Positions: After hemorrhoid surgery, it is advisable to avoid positions that may put pressure on the surgical site. Lying flat on your back may indeed cause discomfort due to pressure on the area. Many patients find it more comfortable to sleep on their side or in a prone position (on their stomach) to alleviate pressure. Using pillows to support your body can also help you find a comfortable position.
3. Dressing Changes: It is not uncommon for the dressing to become stuck to the wound, especially if it has dried out. If the dressing is not coming off easily after soaking, do not force it. Instead, you can try soaking the area again in warm water to help loosen the dressing. If it still does not come off, it is best to consult your healthcare provider for guidance. They may recommend a specific method for removing the dressing or may want to assess the wound themselves.
4. Applying Ointments: If the dressing is still attached to the wound, it is generally not advisable to apply ointments or creams directly on top of it, as this may interfere with the healing process or cause irritation. Once the dressing is removed, you can discuss with your doctor whether applying a topical ointment is appropriate for your situation. Some patients may benefit from using a medicated ointment to promote healing and reduce discomfort.
5. Concerns About the Procedure: It is understandably frustrating to hear that not all aspects of your condition may have been addressed during surgery. Communication is key in the patient-doctor relationship, and it is essential to express your concerns to your surgeon. If you feel that your expectations were not met, consider scheduling a follow-up appointment to discuss your concerns in detail. You have the right to seek clarification about your treatment and to understand the rationale behind the surgical decisions made. If you continue to feel dissatisfied with the care you received, it may be worth seeking a second opinion from another qualified healthcare provider.
In summary, recovery from hemorrhoid surgery can involve discomfort and complications that require careful management. It is crucial to maintain open lines of communication with your healthcare provider, express your concerns, and seek clarification on any aspects of your care that are unclear. Additionally, following post-operative care instructions, including proper wound care and pain management, will be vital in ensuring a smooth recovery. If you have any further questions or if your symptoms worsen, do not hesitate to reach out to your healthcare provider for assistance.
