Postoperative Issues After Hemorrhoid Surgery

Hello Doctor: I have been experiencing symptoms of hemorrhoids for about ten years (occasional bleeding during bowel movements and a small lump at the anal opening).
A few years ago, I consulted a colorectal surgeon who also diagnosed it as hemorrhoids.
Recently, I noticed that the small lump at the anal opening has been getting larger.
About a month ago, I consulted two colorectal surgeons for examination, and both confirmed that I have hemorrhoids.
Doctor A mentioned that the lump at my anal opening was due to a previous anal fissure that had become externalized.
Doctor B noted that I have significant internal hemorrhoids and diagnosed me with "combined internal and external hemorrhoids." Both doctors seemed to agree that surgery would be the best way to eliminate the problem.
I also feel that if surgery can permanently resolve my long-standing discomfort and anxiety, I would prefer to proceed with it.
Therefore, I chose to have surgery with Doctor B (I am in Kaohsiung, and Doctor B is considered an authority in this field).
I underwent the surgery about three weeks ago.
Initially, the pain around the anus was quite severe, and it felt like there were many stitches in the area.
However, over the past week, the pain has improved, and there is almost no bleeding or discharge.
Nevertheless, three weeks post-surgery, there is still some swelling and pain around the anus, and it hurts more during bowel movements or when straining.
When I followed up with Doctor B, he mentioned that the skin has not fully healed yet.
I would like to ask if my recovery is considered normal.
How long does it typically take for complete healing (when there is no longer any swelling or pain)? I am concerned about potential complications.
Thank you for taking the time to respond.

Hello: In response to your question, it is normal to experience pain at the surgical site within the first week after hemorrhoid surgery.
Generally, significant improvement is expected after two weeks, and complete recovery may take about a month.
Based on the symptoms you described, this appears to be a typical postoperative phenomenon and not a complication.
If you wish to recover more quickly, taking warm sitz baths (plain water is sufficient) is highly recommended.
Occasionally, the use of topical ointments can also help alleviate these discomforting symptoms.
Please rest assured and take care of yourself, wishing you a speedy recovery.

After undergoing hemorrhoid surgery, it's common to have concerns about recovery and the potential for complications. Based on your description, it seems that you are experiencing typical postoperative symptoms, but let's delve deeper into what you might expect during your recovery process.

First and foremost, pain and swelling around the surgical site are normal after hemorrhoid surgery. The surgical procedure, whether it was a hemorrhoidectomy or another method, involves cutting away tissue, which can lead to inflammation and discomfort as the area heals. The fact that your pain has decreased over the past week is a positive sign, indicating that your body is beginning to heal. However, lingering pain and swelling three weeks post-surgery can still be within the realm of normal, especially if the surgical site is still in the process of healing.

Typically, the recovery timeline for hemorrhoid surgery can vary significantly from person to person. Most patients can expect to experience some discomfort for several weeks following the procedure. Generally, the initial pain should start to diminish within the first week or two, but complete healing can take anywhere from four to six weeks, or even longer in some cases. During this time, it is crucial to follow your surgeon's postoperative care instructions, which may include dietary changes, stool softeners, and pain management strategies.

Regarding your concerns about potential complications or long-term effects, it is essential to monitor your symptoms closely. If you notice any signs of infection, such as increased redness, swelling, or discharge from the surgical site, or if your pain worsens instead of improving, you should contact your healthcare provider immediately. Additionally, if you experience any new symptoms, such as fever or significant changes in bowel habits, these could warrant further evaluation.

In terms of rehabilitation, while formal physical therapy is not typically required after hemorrhoid surgery, there are several self-care measures you can take to facilitate your recovery. These include:
1. Dietary Adjustments: Eating a high-fiber diet can help prevent constipation and straining during bowel movements, which is crucial for healing. Consider incorporating fruits, vegetables, whole grains, and plenty of fluids into your diet.

2. Sitz Baths: Taking warm sitz baths several times a day can help soothe the area and promote healing. This practice can also alleviate discomfort and reduce swelling.

3. Avoiding Strain: It’s important to avoid heavy lifting and straining during bowel movements. If you experience pain while trying to pass stool, consider using a stool softener or laxative as recommended by your doctor.

4. Follow-Up Appointments: Regular follow-ups with your surgeon are essential to monitor your healing progress. Your doctor can provide guidance on when it is safe to resume normal activities and what to expect as you continue to recover.

In conclusion, while your current symptoms may be concerning, they are not uncommon after hemorrhoid surgery. Recovery can take time, and it is essential to be patient with your body as it heals. If you have any ongoing concerns or if your symptoms do not improve, do not hesitate to reach out to your healthcare provider for further evaluation and reassurance. They can provide personalized advice based on your specific situation and help ensure that your recovery progresses smoothly.
