Urinary incontinence, incomplete bladder emptying, discomfort in the penis and testicles?

I have been experiencing discomfort for several months now.
Initially, I felt discomfort in my testicles and penis, with a stinging sensation in my penis.
After urinating, I still felt the urge to urinate, and shortly after finishing, I would feel like more urine was coming out.
I visited a clinic, and the doctor diagnosed me with a urinary tract infection and prescribed medication that would darken my urine.
I took the medication for over two weeks but did not return for a follow-up, and my parents disposed of the remaining medication.
My condition improved somewhat, but due to certain circumstances, my mood deteriorated, and I wasn't drinking much water, which made things worse.
Now, after urinating, I have to strain, but after pulling up my pants, I still feel like urine is leaking out, and I have no urge to urinate, yet I still feel the need to go.

One strange thing is that I always use toilet paper to wipe after urinating, but my underwear still gets damp afterward.
Sometimes, the sensation of urine coming out is not actually urine.
Once, after taking a shower and finishing urinating, I was playing a game and suddenly became very excited, which resulted in an uncontrollable urge to urinate.
This has been bothering me a lot.
I want to drink more water, but I'm concerned because after urinating, I feel like I can't fully empty my bladder, which leads to damp underwear.
My testicles and penis still often feel uncomfortable, and sometimes my penis appears red.

What should I do? I don't understand why I have such an aversion to my urine and semen.
After school, I have to wipe my chair because I often dampen my underwear after using the restroom.
It's really exhausting.
Drinking more water just leads to more frequent urination, and this issue keeps recurring.

Hello: 1) Your condition may be caused by the foreskin; keep the area dry, ensure adequate moisture, and limit beverage intake to prevent inflammation.
2) Additionally, you should visit a urologist, as it is important to confirm that any previous infections have been fully treated.
3) If there is redness on the penis, you should inform your doctor, as they will need to examine you to determine if there is a skin inflammation causing these symptoms.
Wishing you good health.

More Info

It sounds like you are experiencing a complex set of urological symptoms that are causing you significant distress. Based on your description, you have been dealing with discomfort in your genital area, feelings of incomplete urination, and involuntary leakage, which can be quite challenging to manage. Here’s a breakdown of your situation and some recommendations on how to approach it.

Understanding Your Symptoms
1. Urethral Discomfort and Pain: The discomfort in your testicles and penis, along with the stinging sensation, could be indicative of a few conditions, including urethritis (inflammation of the urethra), prostatitis (inflammation of the prostate), or even a sexually transmitted infection (STI). Since you mentioned that a doctor previously diagnosed you with a urinary tract infection (UTI) and prescribed medication, it’s possible that the initial infection may not have fully resolved or that another issue has developed.

2. Incontinence and Urgency: The feeling of needing to urinate frequently, along with the sensation of not fully emptying your bladder, can be symptoms of urinary incontinence or bladder dysfunction. This can sometimes be exacerbated by anxiety or stress, which you mentioned has been affecting your life. The fact that you feel the urge to urinate even after just having done so, and that you experience leakage, suggests that your bladder may be overactive or that there is some dysfunction in the urinary tract.

3. Psychological Factors: You noted that your mood and emotional state have been impacted, which can significantly affect physical symptoms. Anxiety can lead to increased muscle tension, which may exacerbate urinary issues. Additionally, the discomfort you feel towards your own bodily functions can create a cycle of stress and physical symptoms.

Recommendations
1. Consult a Urologist: Given the persistence and complexity of your symptoms, it would be wise to seek a consultation with a urologist. They can perform a thorough evaluation, which may include urine tests, imaging studies, and possibly a cystoscopy to look at the bladder and urethra directly. This will help rule out any underlying conditions that may require specific treatment.

2. Hydration and Diet: While you mentioned that drinking more water seems to worsen your symptoms, staying adequately hydrated is important for urinary health. You might consider gradually increasing your fluid intake and monitoring how it affects your symptoms. Avoiding irritants such as caffeine, alcohol, and spicy foods can also help reduce bladder irritation.

3. Pelvic Floor Exercises: Engaging in pelvic floor exercises (Kegel exercises) can strengthen the muscles that control urination. These exercises can help improve bladder control and reduce feelings of urgency. A physical therapist specializing in pelvic health can guide you through these exercises.

4. Mental Health Support: Since you mentioned that your emotional state has been affected, consider seeking support from a mental health professional. Therapy can help you manage anxiety and stress, which may, in turn, alleviate some of your physical symptoms.

5. Follow-Up Care: Regular follow-ups with your healthcare provider are essential to monitor your symptoms and adjust treatment as necessary. If you feel that your current provider is not addressing your concerns adequately, seeking a second opinion can be beneficial.

Conclusion
Your symptoms are multifaceted and may require a comprehensive approach that includes medical evaluation, lifestyle adjustments, and psychological support. It’s important to address both the physical and emotional aspects of your condition to achieve the best possible outcome. Don’t hesitate to reach out for help, and remember that you are not alone in dealing with these challenges.
