Epididymitis: Symptoms, Treatment, and Long-term Effects - Urology

Epididymitis

Earlier this year, I developed a urinary tract infection and epididymitis due to sexual activity.
Initially, I experienced abdominal pain and fever, and my testicles became two to three times larger than before, with painful hard lumps.
After continuous treatment, I have returned to the previous size and softness, but the hard lumps have not completely disappeared and are still slightly present.
Up to now, everything seems normal, and there have been no complications.
Will these hard lumps stay with me for life? Will they affect my fertility and sexual function?

Xiao Bao, 20~29 year old female. Ask Date: 2020/06/27

Further observation may show that it will gradually resolve.

Epididymitis is an inflammation of the epididymis, a tube located at the back of the testicle that stores and carries sperm. It can be caused by various factors, including bacterial infections, sexually transmitted infections (STIs), or other non-infectious causes. Understanding the symptoms, treatment options, and potential long-term effects of epididymitis is crucial for managing the condition effectively.

Symptoms of Epididymitis
The symptoms of epididymitis can vary in severity and may include:
1. Pain and Swelling: The most common symptom is pain in the scrotum, which may be accompanied by swelling. The affected testicle may feel larger than usual.

2. Tenderness: The area around the epididymis may be tender to the touch, and there may be a hard lump present.

3. Fever and Chills: Some individuals may experience systemic symptoms such as fever, chills, and malaise.

4. Urinary Symptoms: These can include a burning sensation during urination, increased urgency to urinate, or discharge from the penis.

5. Nausea: In some cases, nausea may accompany the pain.

Treatment Options
The treatment for epididymitis typically depends on the underlying cause:
1. Antibiotics: If the condition is caused by a bacterial infection, antibiotics are the primary treatment. It's essential to complete the full course of antibiotics even if symptoms improve.

2. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate pain and reduce inflammation.

3. Rest and Elevation: Resting and elevating the scrotum can help reduce swelling and discomfort.

4. Cold Compresses: Applying a cold pack to the affected area can also provide relief from pain and swelling.

5. Avoiding Sexual Activity: It is advisable to refrain from sexual activity until the infection has resolved and symptoms have improved.

Long-term Effects
In many cases, individuals recover from epididymitis without any long-term complications. However, some may experience residual effects:
1. Hard Lumps: As you mentioned, it is not uncommon for a hard lump to remain even after the acute inflammation has resolved. This lump may be a result of scar tissue or residual inflammation. While it can be concerning, it does not necessarily indicate a serious problem.

2. Fertility Concerns: Most individuals with epididymitis do not experience long-term fertility issues. However, if the infection is severe or recurrent, it could potentially affect sperm production or transport. Regular follow-ups with a healthcare provider can help monitor any changes in fertility.

3. Sexual Function: Generally, epididymitis does not affect sexual function. However, if there are complications or if the condition is chronic, some individuals may experience discomfort during sexual activity.

Conclusion
It is understandable to be concerned about the hard lump you are experiencing. While it may persist, it is often benign and not indicative of a serious underlying issue. Regular check-ups with your healthcare provider can help monitor the situation and address any concerns you may have regarding fertility or sexual function. If you notice any changes in symptoms or if the lump becomes painful, it is essential to seek medical advice promptly. Overall, with appropriate treatment and monitoring, most individuals with epididymitis can expect a good prognosis and a return to normal function.
