Can I get pregnant?

Hello doctor, on June 13th, I had some friction with my boyfriend while he was wearing underwear and I wasn't.
It lasted for about two thrusts, and then his fingers went inside (he confirmed that there was no semen or only a small amount of pre-ejaculate fluid).
After that, he wore a condom and we had a few more thrusts over the underwear (there was no penetration and no ejaculation).
I would like to know what the chances of pregnancy are in this situation, especially since my period is already 14 days late (my last period was from May 18th to now July 7th).
Thank you for your response!

S, 10~19 year old female. Ask Date: 2020/07/07

Dear S,
After reviewing the entire situation, since you have experienced a 14-day delay in your menstrual cycle, I recommend that you either purchase a home pregnancy test or visit a gynecologist for a pregnancy test.
Generally, it is advised to take a pregnancy test after a 7-day delay in menstruation.
The results of the pregnancy test will be the primary indicator of whether you are pregnant.

The question of whether one can get pregnant from outercourse, particularly in the context you've described, is a nuanced one. Outercourse typically refers to sexual activities that do not involve vaginal penetration, such as mutual masturbation or rubbing against each other while clothed. In your case, the activities involved some degree of genital contact, which raises the question of potential pregnancy risk.

To address your concerns, let's break down the components of your situation:
1. Pre-ejaculate Fluid: The fluid that can be released from the penis before ejaculation, known as pre-ejaculate or pre-cum, can contain sperm. While the concentration of sperm in pre-ejaculate is generally lower than in ejaculate, there is still a possibility of pregnancy if sperm are present. If your boyfriend had any residual sperm on his fingers from previous activities, there is a slight risk of sperm being transferred to your vaginal area.

2. Clothing Barrier: You mentioned that your boyfriend was wearing underwear during the outercourse. While this provides a barrier, it is not foolproof. Sperm can potentially seep through fabric, especially if there is any moisture involved. However, the likelihood of sperm successfully traveling through clothing and leading to pregnancy is significantly lower than with direct genital contact.

3. Timing of Ovulation: The timing of your menstrual cycle is crucial in assessing pregnancy risk. If your last menstrual period started on May 18 and your cycle is typically around 30 days, you would likely have ovulated around June 1-3. If you had unprotected contact around that time, the risk of pregnancy would be higher. However, since you mentioned that your period is now 14 days late, this could indicate a potential pregnancy, especially if you had any unprotected contact near your ovulation window.

4. Pregnancy Testing: Given that your period is late, the most reliable way to determine if you are pregnant is to take a home pregnancy test. These tests are designed to detect the hormone hCG (human chorionic gonadotropin), which is produced after a fertilized egg implants in the uterus. Testing a week after your missed period will yield the most accurate results.

5. Consulting a Healthcare Provider: If you have concerns about potential pregnancy or if you experience any unusual symptoms, it is advisable to consult a healthcare provider. They can provide guidance, perform a pregnancy test, and discuss your options moving forward.

6. Future Considerations: If you want to avoid unintended pregnancies in the future, consider discussing contraceptive options with your partner. There are many effective methods available, including hormonal birth control, condoms, and other barrier methods that can significantly reduce the risk of pregnancy during sexual activities.

In summary, while the risk of pregnancy from the activities you described is relatively low, it is not zero, especially considering the potential for sperm transfer. Given your late period, taking a pregnancy test would be a prudent next step. If you have further questions or concerns, do not hesitate to reach out to a healthcare professional for personalized advice and support.
