Post-circumcision ejaculation?
Dear Doctor,
I am currently three weeks post-circumcision surgery.
The wound seems to be healing well, but there is some swelling.
Today, I couldn't resist the urge to try masturbation, but I was cautious not to touch the wound, so I focused on the base of the penis.
I may have gripped a bit too tightly, and since I was stimulating the base, the sensation wasn't very strong.
The act lasted a bit longer than usual (sometimes it would soften due to insufficient stimulation), but during ejaculation, I felt pain in the middle of the base of the penis, and only a very small amount of semen was released.
However, I did not experience any pain while urinating afterward.
Is this normal? Could it be due to the wound and swelling not fully healing yet? Thank you for your response!

Hello: The surgery should not be related to ejaculation.
If symptoms persist, please consult your surgeon during your follow-up appointment.
Wishing you good health.

Post-circumcision concerns, particularly regarding ejaculation and pain, are not uncommon and can be attributed to several factors related to the healing process. After circumcision, it is essential to understand that the surgical site undergoes a healing phase that can last several weeks, and during this time, various sensations and discomforts may arise.
From your description, it seems that you are currently three weeks post-surgery, and while the wound appears to be healing well, you are experiencing some swelling and pain during ejaculation. It is important to note that some degree of swelling (edema) is typical after circumcision, especially in the initial weeks following the procedure. This swelling can contribute to discomfort during sexual activity, including masturbation and ejaculation.
The pain you experienced at the base of the penis during ejaculation could be due to several reasons:
1. Healing Tissue: The tissues around the circumcision site are still healing. Engaging in sexual activity or masturbation can sometimes irritate the healing tissues, leading to discomfort or pain. The sensitivity of the area may also be heightened during the healing process.
2. Swelling: As you mentioned, there is some swelling present. Swelling can create pressure in the area, which may lead to discomfort during ejaculation. This pressure can also affect the normal flow of semen, potentially resulting in a reduced volume of ejaculate.
3. Technique: You indicated that you were cautious about not touching the surgical site and were gripping the base of the penis. This technique may not provide adequate stimulation, leading to prolonged activity without sufficient arousal, which can contribute to discomfort and difficulty achieving ejaculation.
4. Psychological Factors: Anxiety about the healing process and fear of causing further injury can also affect sexual performance and sensation. This psychological component can lead to tension, which may exacerbate discomfort.
In terms of whether this is normal, it is relatively common to experience some discomfort during the recovery phase after circumcision, especially when engaging in sexual activity. However, if the pain persists or worsens, or if you notice any signs of infection (such as increased redness, discharge, or fever), it is crucial to consult your healthcare provider for further evaluation.
To aid in your recovery and minimize discomfort, consider the following recommendations:
- Avoid Strain: Give yourself time to heal fully before resuming more vigorous sexual activity. It may be beneficial to wait until you are at least six weeks post-surgery before engaging in activities that could strain the area.
- Gentle Stimulation: If you choose to engage in masturbation, consider using a gentler technique that avoids putting pressure on the surgical site. This may help reduce discomfort.
- Ice and Elevation: If swelling persists, applying ice packs (wrapped in a cloth) to the area for short periods can help reduce swelling. Elevating the area may also assist in minimizing edema.
- Follow-Up: Keep your follow-up appointments with your healthcare provider to monitor your healing progress. They can provide personalized advice based on your specific situation.
In summary, while some discomfort during ejaculation post-circumcision can be normal due to healing and swelling, it is essential to listen to your body and seek medical advice if you have concerns. Your healthcare provider can offer guidance tailored to your recovery and help ensure that you heal properly.
