Issues related to the location of cholesteatoma?
Hello Doctor, I would like to ask some questions regarding pearly penile papules.
I have confirmed that the lesions on my coronal sulcus are indeed pearly penile papules, as they have been present for over a decade.
However, I have a few other bumps located on both sides of the frenulum that are irregularly shaped and look almost identical to the pearly penile papules, except they are slightly more raised (possibly because there are only a few of them, making them more noticeable).
These bumps have also been with me for over ten years.
I would like to know if these could also be pearly penile papules, considering they are not located on the coronal sulcus, and over the years, they have neither increased nor decreased in number, nor have they grown or shrunk.
They are also painless and itch-free, which has caused me some concern for a long time.

More Info
Pearly penile papules (PPP) are benign, small, dome-shaped bumps that typically appear around the corona of the glans penis. They are a common anatomical variant and are often mistaken for sexually transmitted infections or other pathological conditions. Understanding the characteristics and implications of PPP can help alleviate concerns for individuals who notice these growths.
First, it’s important to note that PPP is not a disease or a sign of any health issue. They are completely harmless and do not require treatment unless the individual desires cosmetic removal. The presence of PPP is normal and does not indicate any underlying medical condition or infection. They are particularly common in young adult males and can be present for many years, often remaining stable in size and number, as you have described.
Regarding your specific concerns about the bumps located on the sides of the frenulum that appear different from typical PPP, it is essential to consider a few factors. If these bumps have been present for over a decade without any changes in size, shape, or symptoms (such as pain or itching), they are likely benign as well. However, the irregular shape and location may warrant a closer examination by a healthcare professional to rule out other conditions.
Conditions that can sometimes be confused with PPP include:
1. Fordyce Spots: These are small, painless, pale bumps that are sebaceous glands and are also harmless. They can appear on the shaft of the penis and are not a cause for concern.
2. Pearly Penile Papules Variants: While PPP typically appears in a single row around the corona, variations can occur. If the bumps are similar in appearance to PPP but are located in a different area, they may still be benign.
3. Other Dermatological Conditions: Conditions such as molluscum contagiosum, genital warts, or folliculitis can present with similar features but usually have additional symptoms or characteristics that differentiate them from PPP.
If you are experiencing anxiety about these bumps, it is advisable to consult a dermatologist or urologist. They can perform a physical examination and, if necessary, a biopsy to confirm the diagnosis. This can provide peace of mind and ensure that there are no other underlying issues.
In summary, while your description suggests that the bumps are likely benign and similar to PPP, a professional evaluation is the best way to confirm this. Remember that many individuals have variations in their anatomy, and the presence of these bumps does not typically indicate a health problem. If you have any further concerns or if there are changes in the appearance of these bumps, do not hesitate to seek medical advice.
