Redness and itching on the inner side of the foreskin?

Hello Doctor, three days ago while showering, I think I may have been too vigorous, and I felt some pain near the frenulum of the foreskin, possibly due to a tear.
The next day when I woke up, the inner side of the foreskin became red and itchy, and painful, especially around the frenulum, the coronal sulcus, and the urethral opening, which was more severe.
On the third day, it improved slightly, but touching it still causes significant itching, and I feel the urge to scratch.
Should I go to the pharmacy to buy any ointment to apply? Additionally, when not erect, about half of the glans is covered by the foreskin, but it can be easily retracted.
Does this mean I need a circumcision? Thank you, Doctor.

Hello: 1) If there is a skin break, seek medical attention; do not use ointments on your own, and keep the area clean and dry.
2) If there is no difficulty pushing down during an erection and sexual activity is not an issue, then surgery is not necessary.
Wishing you good health.

More Info

Redness and itching inside the foreskin can be concerning and uncomfortable. Based on the symptoms you've described, it seems you may be experiencing a condition known as balanitis, which is an inflammation of the glans (the head of the penis) and can involve the foreskin as well. This condition can be caused by several factors, including irritation from soaps or hygiene products, infections (such as yeast infections or bacterial infections), or even friction from vigorous washing, as you mentioned.

Causes of Redness and Itching Inside the Foreskin
1. Irritation: Overzealous washing or using harsh soaps can irritate the sensitive skin of the foreskin and glans, leading to redness and itching.

2. Infections:
- Yeast Infections: Candida, a type of yeast, can cause inflammation and is more common in individuals who are uncircumcised.

- Bacterial Infections: Certain bacteria can also lead to balanitis, especially if there is a break in the skin.

- Sexually Transmitted Infections (STIs): Some STIs can cause similar symptoms, so it's essential to consider this if you've had unprotected sexual contact.

3. Skin Conditions: Conditions like eczema or psoriasis can also affect the genital area, leading to redness and itching.

4. Allergic Reactions: Allergies to latex (from condoms), personal care products, or even laundry detergents can cause irritation.

Remedies and Treatments
1. Gentle Hygiene: Ensure that you are cleaning the area gently with mild soap and water. Avoid scrubbing too hard, as this can exacerbate irritation.

2. Topical Treatments: Over-the-counter hydrocortisone cream can help reduce inflammation and itching. Antifungal creams may be beneficial if a yeast infection is suspected. However, it's best to consult with a healthcare professional before starting any treatment.

3. Avoid Irritants: Stay away from scented soaps, lotions, or other products that may irritate the area.
4. Loose Clothing: Wearing loose-fitting underwear can help reduce friction and irritation.

5. Consult a Doctor: If symptoms persist or worsen, it's crucial to see a healthcare provider. They can perform an examination, possibly take a culture if an infection is suspected, and provide a more targeted treatment plan.

Regarding Circumcision
You mentioned that when not erect, about half of the glans is covered by the foreskin but can be easily retracted. This is generally considered normal, and circumcision is not necessary unless there are recurrent infections, significant discomfort, or other medical reasons. Many men live comfortably with their foreskin, and as long as you can maintain proper hygiene and there are no complications, circumcision may not be required.

Conclusion
In summary, the redness and itching you're experiencing could be due to irritation, infection, or a skin condition. Gentle care, avoiding irritants, and possibly using topical treatments can help alleviate your symptoms. However, if the condition does not improve or if you have concerns about recurrent issues, seeking medical advice is essential. A healthcare provider can offer a proper diagnosis and treatment plan tailored to your needs.
