Drug interactions between antifungal medications for onychomycosis and analgesics?
I am currently taking Terbinafine oral tablets to treat onychomycosis.
If I also want to take the pain reliever "Yongsheng" Naproxen, will there be any issues? Thank you.

Dr. Hou Fangmin reply Pharmacology
Hello: Lamisil (Terbinafine) for nail fungus does not interact with Naproxen; however, concurrent use of Terbinafine and caffeine may increase adverse effects of caffeine, such as headache, agitation, insomnia, and diuresis.
Additionally, liver function tests should be conducted prior to using Lamisil, and if liver function levels rise during the 4-6 weeks of treatment, Terbinafine should be discontinued immediately.
Wishing you good health and a Merry Christmas!

More Info
When considering the interactions between antifungal medications, such as Terbinafine (the active ingredient in "療黴舒" or Lamisil), and pain relievers like "永勝" (which contains Ibuprofen), it is essential to understand both the pharmacological properties of these medications and their potential interactions.
Antifungal Medications: Terbinafine
Terbinafine is an antifungal medication primarily used to treat fungal infections of the skin and nails, including onychomycosis (fungal nail infection). It works by inhibiting the synthesis of ergosterol, an essential component of fungal cell membranes, leading to cell death. Terbinafine is generally well-tolerated, but it can have side effects, including gastrointestinal disturbances, liver enzyme elevation, and skin reactions.
Pain Relievers: Ibuprofen
Ibuprofen is a non-steroidal anti-inflammatory drug (NSAID) that is commonly used to relieve pain, reduce inflammation, and lower fever. It works by inhibiting the production of prostaglandins, which are chemicals in the body involved in inflammation and pain signaling. While Ibuprofen is effective for pain relief, it can also have side effects, particularly on the gastrointestinal tract and kidneys, especially when used in high doses or for prolonged periods.
Potential Interactions
1. Liver Function: Both Terbinafine and Ibuprofen can affect liver function, although they do so through different mechanisms. Terbinafine is metabolized in the liver, and its use can lead to elevated liver enzymes. If you are taking Terbinafine, it is crucial to monitor liver function, especially if you are also taking other medications that can affect the liver. While Ibuprofen is not primarily metabolized by the liver, it can still contribute to liver strain when taken in conjunction with other medications that affect liver function.
2. Gastrointestinal Effects: Ibuprofen can cause gastrointestinal irritation, ulcers, or bleeding, particularly in individuals who have a history of gastrointestinal issues or are taking other medications that can exacerbate these effects. If you are taking Terbinafine, which can also cause gastrointestinal side effects, combining these medications may increase the risk of gastrointestinal discomfort.
3. Kidney Function: Both medications can impact kidney function, particularly in individuals with pre-existing kidney issues or those who are dehydrated. Ibuprofen can cause acute kidney injury, especially in high doses or prolonged use. If you are taking Terbinafine, it is essential to ensure adequate hydration and monitor kidney function, particularly if you are also using Ibuprofen for pain relief.
Recommendations
- Consult Your Healthcare Provider: Before combining Terbinafine with Ibuprofen, it is advisable to consult your healthcare provider or pharmacist. They can provide personalized advice based on your medical history, current medications, and overall health status.
- Monitor for Side Effects: If you do take both medications, be vigilant for any signs of liver dysfunction (such as jaundice, dark urine, or abdominal pain), gastrointestinal issues (such as stomach pain, nausea, or blood in stools), or kidney problems (such as decreased urine output or swelling).
- Use the Lowest Effective Dose: If your healthcare provider approves the combination, use the lowest effective dose of Ibuprofen for the shortest duration necessary to manage your pain.
In summary, while there may not be direct contraindications for using Terbinafine and Ibuprofen together, it is crucial to be aware of their potential interactions and side effects. Always consult with a healthcare professional before starting any new medication, especially when combining treatments.
