A few questions..?
I have a few questions for the doctor: 1.
When I urinate, the stream splits, and it’s quite severe, causing urine to splash everywhere.
Can this be treated? 2.
I experience pain when urinating after masturbation, but I don’t have pain during regular urination.
Is this urethritis? 3.
How can I determine if my foreskin is too long? I can barely retract it for cleaning when erect, and I find it inconvenient to clean every two to three days, so I’m considering circumcision.
If it’s not too long, can I still get circumcised? Will I need to take medication after the surgery?

1.
If there is no urethral stricture, there is no need for treatment for split urine flow.
2.
After ejaculation, the urethral muscles may experience discomfort due to intense contractions, which is normal and not a cause for concern.
3.
The foreskin is a natural condition; if it can be retracted and cleaned daily, surgery is not necessary.
However, if you experience issues every two to three days, it is advisable to visit a urologist for an evaluation to determine if circumcision is needed.

When it comes to urological issues, many patients have common concerns that can significantly affect their quality of life. Let's address your questions one by one.
1. Urination Issues (Splitting Stream): The phenomenon of a split urine stream can be caused by several factors. One common reason is the presence of a urethral stricture, which is a narrowing of the urethra that can obstruct urine flow. Other potential causes include an enlarged prostate in males or even anatomical variations. Treatment options depend on the underlying cause. If a stricture is diagnosed, procedures such as urethral dilation or urethrotomy may be necessary. It's important to consult a urologist for a proper evaluation, which may include a physical examination and possibly imaging studies.
2. Pain During Urination After Masturbation: Experiencing pain during urination after masturbation but not at other times could indicate a few possibilities. One common cause is urethritis, which is inflammation of the urethra often due to infection. It could also be related to irritation from the act of masturbation itself, especially if there is excessive friction or if lubrication is not used. If the pain persists or is accompanied by other symptoms such as discharge or fever, it is crucial to seek medical attention. A urologist can perform tests to determine if there is an infection or another underlying issue that needs to be addressed.
3. Concerns About Foreskin Length and Circumcision: The definition of "overlong" foreskin can vary, but generally, if you can retract the foreskin to clean the glans without discomfort, it may not be considered problematic. However, if you find it difficult to maintain hygiene or if you experience recurrent infections or discomfort, circumcision could be a viable option. Many individuals choose circumcision for personal, cultural, or medical reasons. If you decide to proceed with the surgery, it is typically a straightforward procedure, and most patients do not require extensive medication post-operatively. Pain relief is usually managed with over-the-counter medications, but your doctor will provide specific instructions based on your individual case.
In summary, each of these issues can be effectively managed with the right medical guidance. It is essential to consult a urologist who can provide a thorough evaluation and recommend appropriate treatments based on your specific symptoms and medical history. Don't hesitate to seek help, as many urological conditions are common and treatable.
