Femoral fracture
Hello, Doctor You.
I suffered a thigh fracture and a hip fracture from a car accident, and I have already undergone surgery.
They used intramedullary nails for fixation.
It has been two and a half months since the surgery on October 30.
Shortly after the surgery, I noticed a discrepancy in leg length, with a difference of almost 2 centimeters.
Is there a chance for this issue to improve? Can the intramedullary nails be removed to allow for further lengthening of the bone? Additionally, my right leg feels stiff when I try to lay it flat.
I have not undergone any rehabilitation yet.

Hello: The removal of bone screws generally does not cause the "lengthening" of bones as you mentioned.
However, it is important to discuss with your attending physician whether there is indeed a condition of leg length discrepancy.
If there is, we need to determine the exact difference.
Once we have a scientifically valid answer (through imaging), we can provide you with some sound advice.
Thank you.

Leg length discrepancy (LLD) after a femur fracture can be a concerning issue for many patients. In your case, having sustained a femur and hip fracture that required surgery with intramedullary nailing, it is understandable that you are worried about the potential for a noticeable difference in leg length, which you have already observed to be approximately 2 centimeters.
Firstly, it is important to recognize that leg length discrepancies can occur for several reasons following a fracture. These reasons may include the way the bones heal, the surgical technique used, and the presence of any complications during the healing process. In some cases, the healing process can lead to a shortening of the bone, while in others, it may result in overgrowth or malalignment.
Regarding your specific situation, there are a few key points to consider:
1. Potential for Improvement: In some cases, leg length discrepancies can improve over time as the body continues to heal and adapt. However, the extent of improvement largely depends on the individual’s healing process and the specific nature of the fracture. If the discrepancy is due to the way the bone has healed, it may not fully correct itself without intervention.
2. Intramedullary Nail Removal: The removal of the intramedullary nail is a common consideration in cases of leg length discrepancy. However, this procedure is typically performed after the bone has fully healed, which can take several months. Removing the nail too early can risk the stability of the fracture site. Once the bone has healed adequately, your orthopedic surgeon can evaluate whether nail removal could potentially allow for further growth or correction of the discrepancy.
3. Rehabilitation: Engaging in a structured rehabilitation program is crucial for recovery after femur surgery. Physical therapy can help improve mobility, strength, and function of the leg. It may also assist in addressing any compensatory movements that arise due to the leg length discrepancy. While you mentioned that you have not yet started rehabilitation, it is highly recommended to do so as soon as your doctor approves it. A physical therapist can provide exercises tailored to your needs, which may help improve your overall leg function and potentially mitigate some of the effects of the discrepancy.
4. Monitoring and Follow-Up: Regular follow-up appointments with your orthopedic surgeon are essential. They can monitor the healing process, assess the leg length discrepancy, and determine the best course of action moving forward. If the discrepancy remains significant after healing, they may discuss options such as limb lengthening procedures or orthotic devices to help balance the legs.
5. Long-Term Considerations: It is also important to consider the long-term implications of a leg length discrepancy. If the difference persists, it may lead to issues such as joint pain, gait abnormalities, or increased risk of osteoarthritis in the affected leg. Addressing the discrepancy early on can help prevent these complications.
In summary, while there is a possibility for improvement in leg length discrepancy after femur fracture surgery, it is essential to engage in rehabilitation, follow up with your orthopedic surgeon, and discuss the potential for intramedullary nail removal at the appropriate time. Each case is unique, and your healthcare team will be best positioned to provide personalized advice based on your specific circumstances.
