After the removal of the metal pins from a femoral fracture, the leg has become longer?
Hello Doctor, I fractured my left thigh in a car accident and underwent surgery to remove the metal pins on January 20th of this year.
My condition has been good, but after being discharged, I noticed that my left thigh seems to be longer, and I am unable to stand straight or walk normally.
My leg has not completely reduced swelling yet.
I would like to know if this is related to bruising or if it is an issue with the bone.
I only realized this after being discharged and couldn't ask my attending physician immediately.
Thank you.

Hello Ms.
Chi, a femoral fracture may potentially lead to complications such as changes in bone length, either elongation or shortening.
However, there is no need to worry excessively.
If there was no noticeable leg length discrepancy prior to the removal, the length of the bone should remain unchanged after the removal of the screws.
Follow-up in the outpatient clinic will suffice.

Leg length discrepancy (LLD) after femur fracture surgery can be a concerning issue for many patients. In your case, it appears that you have experienced a perceived lengthening of your left thigh following the removal of the fixation hardware. This situation can arise from several factors, including the healing process, soft tissue changes, and the surgical technique used.
Firstly, it is essential to understand that after a femur fracture, the body undergoes a complex healing process. This process involves not only the bone but also the surrounding soft tissues, including muscles, tendons, and ligaments. When a fracture occurs, there can be significant swelling and bruising (hematoma formation) in the area, which may temporarily affect the perceived length of the limb. As the swelling subsides, the soft tissues may return to their normal state, potentially altering the alignment and appearance of the limb.
Secondly, the surgical technique used during your operation can also play a role in the final outcome. If the fracture was stabilized with intramedullary nails or plates, the alignment of the bone fragments during surgery is crucial. If there was any malalignment during the fixation, it could lead to a discrepancy in leg length. Additionally, if the bone was compressed or distracted during the healing process, this could also affect the final length of the femur.
Another factor to consider is the possibility of muscle imbalances or weakness following surgery. After a significant injury and subsequent surgery, it is common for certain muscle groups to become weaker or tighter than others. This can lead to altered biomechanics and may make it challenging to stand straight or walk normally. Physical therapy is often recommended to address these issues, focusing on strengthening the muscles around the hip and knee and improving overall function.
In your case, since you mentioned that you are still experiencing swelling, it is possible that the soft tissue changes are contributing to your perception of leg length discrepancy. As the swelling resolves, you may find that your ability to stand straight and walk normally improves. However, it is crucial to monitor your symptoms closely and communicate with your healthcare provider about any ongoing concerns.
If the discrepancy persists or worsens, further evaluation may be necessary. This could include imaging studies, such as X-rays, to assess the alignment of the femur and ensure that there are no complications, such as nonunion or malunion of the fracture. Your doctor may also consider referring you to a physical therapist who specializes in post-surgical rehabilitation to help you regain strength and function in your leg.
In summary, leg length discrepancy after femur fracture surgery can result from various factors, including soft tissue swelling, surgical technique, and muscle imbalances. It is essential to follow up with your healthcare provider to discuss your concerns and receive appropriate guidance on rehabilitation and recovery. With proper management, many patients can achieve a satisfactory outcome and return to their normal activities.
