Leg Length Discrepancy After Femur Fracture Surgery - Rehabilitation

Femoral fracture

I was in a car accident on February 9th at 11 PM, and I underwent surgery the next day, February 10th, around 3 PM.
I have a closed fracture of my right femur.
It has been a week since the surgery, and I have noticed that my left and right knees are misaligned by about 2 to 3 centimeters, with my right leg appearing longer than my left.
Prior to the accident, my left knee already had a slight inward angle, but there was no significant difference compared to my right knee.
Why has the discrepancy become so pronounced after the accident and surgery? Additionally, why does my right buttock hurt when I lie flat for an extended period at night?

Understanding leg length discrepancy after a femur fracture surgery is a common concern among patients who have experienced significant trauma to the leg. In your case, the discrepancy of 2 to 3 centimeters between your left and right legs following a closed femur fracture is indeed noteworthy and can be attributed to several factors.

Firstly, it is essential to understand that fractures, especially in the femur, can lead to changes in the alignment and length of the bones. When a fracture occurs, the body’s natural healing process involves the formation of a callus around the fracture site. This callus is made of new bone tissue that can sometimes lead to a slight increase in the length of the bone. However, if the fracture is not properly aligned during surgery, it can result in a permanent leg length discrepancy. This misalignment can occur due to various reasons, including the positioning of the leg during surgery, the type of fixation used, and the surgeon's technique.

In your situation, the fact that you had a pre-existing condition of "internal rotation" of the left knee may have contributed to the perception of leg length discrepancy. The internal rotation could have altered the alignment of your knees, making it more pronounced after the trauma and subsequent surgery. The trauma from the accident may have exacerbated this condition, leading to a more noticeable difference in leg length.

Regarding the pain you experience in your right hip when lying down, this could be attributed to several factors. After a significant injury and surgery, it is common for patients to experience discomfort in the hip and surrounding areas due to muscle tightness, changes in gait, or compensatory movements. When one leg is longer or feels different, it can lead to an altered walking pattern, which may put additional strain on the hip and lower back, causing pain. Additionally, lying in one position for an extended period can lead to pressure on certain areas, resulting in discomfort.

To address your concerns about leg length discrepancy, it is crucial to follow up with your orthopedic surgeon. They may recommend imaging studies, such as X-rays, to assess the alignment of your femur and determine if there is any significant displacement or malunion. If a discrepancy is confirmed, treatment options may include physical therapy to strengthen the muscles around the hip and knee, or in some cases, surgical intervention may be necessary to correct the alignment.

In the meantime, engaging in physical therapy can be beneficial. A physical therapist can provide exercises to improve strength, flexibility, and range of motion, which may help alleviate some of the discomfort you are experiencing. They can also guide you on proper body mechanics to minimize strain on your hip and lower back.

In conclusion, leg length discrepancy after femur fracture surgery can arise from various factors, including surgical alignment and pre-existing conditions. It is essential to communicate your concerns with your healthcare provider, who can offer tailored advice and treatment options to help you recover effectively. Remember that recovery from such injuries takes time, and addressing any discrepancies early on can lead to better long-term outcomes.

It is essential to assess whether there is a difference in length after the fracture has healed. If there is no difference, it may be related to muscle atrophy. Muscle training can be beneficial, and using orthotic insoles is also an option.
