Right upper quadrant pain?
Hello, doctor.
For the past two months, I have occasionally experienced sharp, stabbing pain in the area under my right rib cage, about 3 to 4 times a day.
Sometimes, I also feel a dull pain just above my navel on the right side.
Could you please help me understand what might be causing this? Thank you.

It may be neuropathic pain; local heat application can be helpful.
If there is no improvement, it is advisable to seek medical evaluation.

Right upper abdominal pain can be a complex issue, as it can stem from various underlying causes. Given your description of intermittent sharp and dull pain in the right upper quadrant, particularly near the rib cage and around the area of the navel, it is essential to consider several potential diagnoses.
1. Gallbladder Issues: One of the most common causes of right upper abdominal pain is related to the gallbladder, particularly gallstones or cholecystitis (inflammation of the gallbladder). Symptoms often include sharp pain after eating fatty meals, nausea, and sometimes jaundice if a stone obstructs the bile duct.
2. Liver Conditions: Given your history of chronic hepatitis B, liver-related issues could also be a concern. Conditions such as fatty liver disease, liver cirrhosis, or even liver tumors can present with pain in the right upper abdomen. Monitoring liver function tests regularly is crucial, especially since you have a history of acute hepatitis.
3. Gastrointestinal Disorders: Conditions like gastritis, peptic ulcers, or even irritable bowel syndrome (IBS) can cause discomfort in the upper abdomen. Symptoms may vary from sharp pains to a feeling of fullness or pressure.
4. Musculoskeletal Pain: Sometimes, pain in this area can be due to muscle strain or rib issues, especially if there has been any recent physical activity that could have led to a strain. This type of pain often worsens with movement or certain positions.
5. Pancreatitis: Although less common, inflammation of the pancreas can cause pain in the upper abdomen, often radiating to the back. This condition is usually accompanied by other symptoms such as nausea and vomiting.
6. Kidney Issues: Right-sided kidney stones or infections can also present as pain in the right upper abdomen, often accompanied by flank pain or changes in urination.
7. Referred Pain: Sometimes, pain in the right upper abdomen can be referred from other areas, such as the heart or lungs, particularly if there are underlying conditions affecting these organs.
Given the complexity of abdominal pain and the potential for serious underlying conditions, it is crucial to seek medical attention if the pain persists or worsens. A thorough evaluation by a healthcare professional, including a detailed history, physical examination, and possibly imaging studies (like an ultrasound or CT scan), will help determine the exact cause of your symptoms.
In the meantime, keeping a symptom diary may be beneficial. Note when the pain occurs, its intensity, any associated symptoms (like nausea, changes in appetite, or bowel habits), and any activities that seem to trigger or alleviate the pain. This information can provide valuable insights for your healthcare provider.
Lastly, considering your anxiety, it's important to acknowledge that stress and anxiety can exacerbate physical symptoms, including abdominal pain. Techniques such as mindfulness, relaxation exercises, and cognitive behavioral therapy may help manage anxiety and its physical manifestations. However, addressing the underlying cause of your abdominal pain should remain a priority.
