Persistent pain in the right upper arm?
I would like to ask the doctor, I am currently 18 years old, but I have had persistent pain in my right upper arm since childhood.
It is not particularly severe, but certain specific positions can trigger intense soreness, requiring me to change my posture to alleviate it slightly.
This issue often occurs during sleep; for example, I cannot sleep on my left side as it causes significant soreness in my right upper arm, making it difficult to fall asleep.
Sometimes, I can relieve the pain by raising my arm, but as soon as I return to my original position, the soreness returns.
I have researched many articles online, but since this has been an issue since I was young, I am unsure if it is caused by poor posture or perhaps related to writing or sitting habits.
The primary symptom is soreness in my right upper arm.
I would like to know if there are any underlying causes for these symptoms or if there are ways to address them.

Hello: Based on your description, it may be necessary to consider cervical spine disorders, possibly due to nerve root compression.
Excessive force during cervical rotation and other external factors may lead to cervical instability, dislocation, and cervical disc herniation.
Typical discomfort symptoms associated with cervical nerve root pathology include pain in the shoulder blade or arm, as well as numbness, tingling, and weakness in the palm and fingers.
Treatment can be divided into medication management: When nerves are compressed, an inflammatory response occurs, leading to symptoms such as pain, numbness, and weakness.
Non-steroidal anti-inflammatory drugs (NSAIDs) can be used for treatment, as they have both anti-inflammatory and analgesic effects, which can reduce the inflammatory response and alleviate pain.
Physical therapy includes:
1.
Heat therapy: Primarily increases local blood circulation and improves muscle ischemia.
2.
Shortwave and ultrasound therapy: Mainly promotes blood circulation in deeper tissues.
3.
Transcutaneous electrical nerve stimulation (TENS) and medium-frequency vector interference current: Can provide local pain relief and alleviate tension and discomfort in the neck and shoulders.
4.
Cervical traction: Commonly used for degenerative cervical spondylosis, disc herniation (cervical osteophytes), with concurrent nerve root compression, and cervical injuries without fractures or joint instability, where there is nerve compression.
Cervical traction can be utilized for rehabilitation, but X-ray examination is required beforehand to confirm joint stability.
Cervical collar stabilization: Prevents cervical instability and avoids further serious nerve tissue damage.
It is suitable for injuries such as sports injuries, car accidents, or falls that lead to excessive flexion or extension of the cervical spine.
I recommend that you consult with a neurosurgeon or a rehabilitation physician to discuss examination and treatment options.
Wishing you good health and peace.

Chronic right upper arm pain, especially when it has been persistent since childhood, can be quite concerning and may stem from various underlying causes. Given your age and the nature of your symptoms, it is essential to consider both musculoskeletal and postural factors that could be contributing to your discomfort.
Possible Causes
1. Muscle Strain or Overuse: Repetitive activities, such as writing or using a computer, can lead to muscle strain. If you frequently engage in activities that require repetitive arm movements, this could result in chronic muscle fatigue and discomfort.
2. Poor Posture: Poor posture, especially during activities like sitting or sleeping, can lead to muscle imbalances and strain. If you tend to slouch or have your shoulders rounded forward, this can place additional stress on the muscles of the upper arm and shoulder.
3. Nerve Compression: Conditions such as thoracic outlet syndrome or cervical radiculopathy can cause pain that radiates into the arm. This can occur if nerves in the neck or shoulder area are compressed, leading to symptoms like pain, tingling, or weakness.
4. Rotator Cuff Issues: Although you are young, rotator cuff injuries can occur due to overuse or repetitive strain. This could lead to inflammation and pain in the shoulder that may radiate down the arm.
5. Tendinitis: Inflammation of the tendons in the shoulder or upper arm can cause chronic pain. This is often seen in individuals who engage in repetitive overhead activities.
6. Referred Pain: Sometimes, pain in the arm can be referred from other areas, such as the neck or upper back. Conditions like cervical spondylosis or herniated discs can cause referred pain that manifests in the arm.
Management Strategies
1. Postural Correction: Focus on maintaining good posture while sitting, standing, and sleeping. Ergonomic adjustments to your workspace can help reduce strain on your upper body. Consider using a chair that supports your back and keeps your shoulders relaxed.
2. Physical Therapy: A physical therapist can assess your posture and movement patterns, providing exercises to strengthen weak muscles and stretch tight ones. They can also teach you techniques to alleviate pain during specific activities.
3. Activity Modification: If certain activities exacerbate your pain, try to modify them. For example, take frequent breaks when writing or using a computer, and incorporate stretches to relieve tension in your upper body.
4. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage pain and inflammation. However, consult with a healthcare provider before starting any medication.
5. Heat and Cold Therapy: Applying heat can help relax tight muscles, while cold packs can reduce inflammation. Alternating between the two may provide relief.
6. Strengthening Exercises: Engage in exercises that strengthen the muscles around your shoulder and upper arm. This can help improve stability and reduce the risk of injury.
7. Consultation with a Specialist: If your symptoms persist despite conservative management, consider consulting an orthopedic specialist or a sports medicine physician. They may recommend imaging studies, such as an MRI, to rule out any underlying structural issues.
Conclusion
Chronic right upper arm pain can be multifactorial, and addressing it requires a comprehensive approach. By focusing on posture, engaging in physical therapy, and modifying activities, you can significantly improve your symptoms. If pain persists, seeking further evaluation from a healthcare professional is crucial to rule out any serious underlying conditions. Remember, early intervention can lead to better outcomes and prevent the development of chronic pain.
