Right upper arm pain?

Dear Dr.
Fan,
I have been experiencing discomfort in my right upper arm near the shoulder (but not the shoulder itself) for about five months now.
I feel as if a muscle or tendon may be injured.
Sometimes the pain is so severe that when I take a shower and need to remove my clothes, I have to cross my arms in front of me to pull my shirt up.
However, when I get to about chest level, the pain in my right upper arm prevents me from lifting it any higher.
If I force it, I worry that I might tear a muscle or tendon.
Additionally, I have difficulty holding a microphone with my right hand, as it causes the same pain in my right upper arm.
After using the restroom, I also struggle to use my right hand to wipe with toilet paper because of the pain in the area near my shoulder (not the shoulder itself) and the inner side of my right upper arm (not the armpit).
I usually sleep on my right side at night, and whenever I do, my arm feels extremely stiff the next day, making it impossible to lift it backward, and the aforementioned issues occur as well.
Therefore, I try to avoid sleeping on my right side.
If I don’t sleep on my right side the night before, the next day is not as severe (I can remove my clothes and lift my arm backward).
My job involves extensive computer keying, where I have to continuously input stock trading orders from 8:30 AM to 1:30 PM, using my right hand to repeatedly press the numeric keypad.
I have been doing this job for almost two years without any issues, but since March or April of this year, my arm has been like this.
I am unsure if it is related to my work.
Could you please help me understand why my right upper arm is experiencing these problems? (I have not seen any doctors about this issue so far.)

Ms. Chen, 20~29 year old female. Ask Date: 2007/08/12

Understanding right upper arm pain, particularly when it is localized near the shoulder and has persisted for several months, can be quite complex. Based on your description, it seems that you are experiencing significant discomfort that affects your daily activities, including dressing and using your right arm for various tasks. This situation can be frustrating and concerning, especially since you have not yet consulted a physician.

There are several potential causes for the pain you are experiencing in your right upper arm. Given the location and nature of the pain, it could be related to a few common conditions:
1. Rotator Cuff Injury: The rotator cuff is a group of muscles and tendons that stabilize the shoulder and allow for a wide range of arm movements. An injury to this area can lead to pain that radiates down the arm, especially during overhead activities or lifting. Symptoms often include weakness, pain during movement, and difficulty sleeping on the affected side.

2. Tendinitis: Inflammation of the tendons in the shoulder or upper arm can cause pain, particularly with repetitive movements. This is common in individuals who perform repetitive tasks, such as typing or using a mouse for extended periods, which seems to align with your work as a stock trader.

3. Bursitis: Inflammation of the bursa (a small fluid-filled sac that reduces friction between tissues) can also lead to pain in the shoulder and upper arm. This condition is often exacerbated by repetitive overhead movements or prolonged pressure on the shoulder.

4. Muscle Strain: Given your description of pain when performing certain movements, it is possible that you have strained a muscle in your upper arm or shoulder area. This could result from overuse, especially considering your work involves extensive computer use.

5. Nerve Impingement: Sometimes, nerve compression in the neck or shoulder can lead to pain that radiates down the arm. This could be related to poor posture, especially if you are sitting for long periods while working.

To address your pain, consider the following recommendations:
- Rest and Modify Activities: It is crucial to give your arm time to heal. Try to avoid activities that exacerbate the pain, especially those that involve lifting or reaching overhead.

- Ice Therapy: Applying ice to the affected area can help reduce inflammation and numb the pain. Aim for 15-20 minutes of ice application several times a day.

- Over-the-Counter Pain Relief: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen or naproxen can help alleviate pain and reduce inflammation. However, consult with a healthcare provider before starting any medication.

- Physical Therapy: A physical therapist can provide exercises tailored to your condition, focusing on strengthening the shoulder and improving flexibility. They can also teach you proper ergonomics for your work environment to prevent further strain.

- Posture Correction: Since your work involves prolonged computer use, ensure that your workstation is ergonomically set up. Your chair, desk, and monitor should be positioned to minimize strain on your shoulders and arms.

- Consult a Healthcare Professional: Since your symptoms have persisted for several months, it is advisable to seek a medical evaluation. A healthcare provider can perform a physical examination, possibly order imaging studies (like an MRI), and provide a definitive diagnosis and treatment plan.

In summary, while there are several potential causes for your right upper arm pain, it is essential to take proactive steps to manage your symptoms and seek professional guidance. Early intervention can prevent further complications and help you return to your normal activities more quickly.
