Red spots

There are two to three circular red spots on the skin of my palm (they are not peeling, itchy, or painful) that have persisted for over two months without improvement.
I have been applying a topical cream for more than a week without any effect.
I would like to know what could be the cause.
Thank you, doctor!

More Info

Persistent red spots on the skin can be concerning, especially when they last for an extended period without any accompanying symptoms such as itching, pain, or peeling. Based on your description of circular red spots on your palms that have persisted for over two months, several potential causes could be considered.

Possible Causes
1. Dermatitis: Contact dermatitis can occur due to exposure to irritants or allergens. Even if the spots are not itchy or painful, they may still represent a mild inflammatory response.

2. Psoriasis: While typically associated with scaling and itching, psoriasis can sometimes present as red spots without the classic symptoms. It is a chronic autoimmune condition that can affect various parts of the body, including the palms.

3. Eczema: Similar to dermatitis, eczema can manifest as red spots and may not always be itchy. It can be triggered by environmental factors, stress, or dietary changes.

4. Vascular Lesions: Conditions such as cherry angiomas or other benign vascular lesions can appear as red spots on the skin. These are usually harmless and may not require treatment unless they become bothersome.

5. Infections: Certain viral or bacterial infections can cause localized redness. However, the lack of pain or other systemic symptoms makes this less likely.

6. Pityriasis Rosea: This is a self-limiting skin condition that often starts with a single "herald patch" followed by smaller spots. It typically resolves on its own but can sometimes be mistaken for other conditions.

7. Keto Rash: If you have recently undergone significant dietary changes, such as fasting or a ketogenic diet, you might experience keto rash, which can present as red spots on the skin.

Treatment Options
Since you mentioned that you have already tried a topical treatment (膚喜定) for over a week without improvement, it may be time to consider other options:
1. Topical Corticosteroids: If inflammation is suspected, a mild topical corticosteroid may help reduce redness and inflammation. However, this should be done under the guidance of a healthcare provider.

2. Moisturizers: Keeping the skin well-hydrated can help improve the barrier function and may alleviate some symptoms if dermatitis or eczema is involved.

3. Avoid Irritants: Identify and avoid any potential irritants or allergens that may be contributing to the condition. This includes harsh soaps, detergents, or certain fabrics.

4. Consult a Dermatologist: Given the persistence of the symptoms, it would be advisable to consult a dermatologist for a thorough evaluation. They may perform a skin biopsy or other diagnostic tests to determine the exact cause of the red spots.

5. Lifestyle Modifications: If dietary changes are suspected to be a trigger, consider maintaining a balanced diet and avoiding extreme dietary restrictions unless medically supervised.

6. Phototherapy: In some cases, light therapy may be beneficial, especially for conditions like psoriasis or eczema.

Conclusion
In summary, while there are several potential causes for the persistent red spots on your palms, a definitive diagnosis can only be made through a clinical evaluation. It is crucial to consult with a healthcare professional who can assess your condition in person and recommend appropriate treatment options tailored to your specific needs. In the meantime, keeping the affected area moisturized and avoiding known irritants may provide some relief.
