Can Aisilan and Leminada be taken together?

Hello, recently my doctor prescribed me Lamictal (lamotrigine) for bipolar disorder, but I found that this medication may interact with my birth control pill (Alesse).
Can you tell me if it is safe to take both medications together?

V, 20~29 year old female. Ask Date: 2021/07/22

Dr. Weng Yiji reply Pharmacology

Hello: Aislean belongs to the class of estradiol preparations.
Estradiol is primarily metabolized by the liver, and Lamictal (lamotrigine) is also metabolized by the liver.
Therefore, when used together, estradiol may increase the metabolic rate of Lamictal in the liver, leading to a reduced therapeutic effect of Lamictal.
It is recommended that you consult with a gynecologist, inform them that you are currently taking Lamictal, and ask for guidance on selecting a suitable contraceptive pill for you.
I hope this helps!

When considering the concurrent use of Lamictal (lamotrigine) and Alesse (a combination oral contraceptive containing ethinyl estradiol and levonorgestrel), it is essential to understand the potential interactions and their implications for treatment efficacy and safety.

Lamictal is primarily used to treat bipolar disorder and epilepsy. It is metabolized in the liver, and its levels can be affected by various factors, including other medications. Alesse, on the other hand, is a hormonal contraceptive that contains ethinyl estradiol, which is also metabolized by the liver. The concern arises from the fact that certain medications can induce or inhibit liver enzymes, particularly the cytochrome P450 system, which can lead to altered levels of drugs in the bloodstream.

Research indicates that hormonal contraceptives like Alesse can potentially reduce the effectiveness of Lamictal. This is because the estrogen component in Alesse may increase the metabolism of Lamictal, leading to lower plasma concentrations of the drug. Consequently, this could diminish the therapeutic effects of Lamictal, increasing the risk of mood episodes or seizures in individuals who rely on this medication for stability.

Conversely, Lamictal does not significantly affect the efficacy of Alesse. The primary concern is that if Lamictal levels drop due to the interaction with Alesse, it could lead to a resurgence of bipolar symptoms or seizures, which could be detrimental to the patient's health.

Given these considerations, it is crucial for patients taking Lamictal and Alesse to consult with their healthcare provider. A healthcare professional may recommend monitoring Lamictal levels more closely or adjusting the dosage of Lamictal to ensure that therapeutic levels are maintained while using Alesse. In some cases, alternative contraceptive methods that do not interact with Lamictal may be considered, such as non-hormonal options or progestin-only contraceptives, which have a lower risk of interaction.

In summary, while it is possible to take Lamictal and Alesse together, it is essential to do so under medical supervision. Patients should communicate openly with their healthcare providers about all medications they are taking, including over-the-counter drugs and supplements, to ensure safe and effective treatment. Regular follow-ups and possible adjustments to medication regimens can help mitigate any risks associated with these drug interactions, ensuring both effective mood stabilization and reliable contraception.
