Thinning of the foreskin?
Hello, a few years ago I experienced itching under the foreskin, and I applied a topical ointment called Fuhunkang several times.
Later, the skin of the foreskin became thin and is now prone to tearing and bleeding.
What should I do now?

Dr. Zheng Lizhen reply Dermatology
Hello! Based on your description, it is possible that the thinning of the skin and the dilation of blood vessels are due to the side effects of the medication.
However, an in-person consultation is necessary to confirm the actual condition.
Best wishes for your health, Dr.

Thinning foreskin, also known as phimosis or skin atrophy of the foreskin, can be a concerning condition that may lead to discomfort, pain, or even bleeding. It’s essential to understand the potential causes and treatment options available to address this issue effectively.
Causes of Thinning Foreskin
1. Chronic Inflammation: Conditions such as balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin) can lead to skin changes, including thinning. Chronic irritation from infections or skin conditions can also contribute to this problem.
2. Skin Conditions: Conditions like eczema, psoriasis, or lichen sclerosus can cause thinning of the skin. Lichen sclerosus, in particular, is known to cause white patches and thinning of the skin, which can lead to scarring and complications.
3. Overuse of Topical Steroids: While topical steroids can be effective for treating inflammation, overuse can lead to skin thinning. If you have been using a product like hydrocortisone cream (膚潤康) excessively, this could be a contributing factor.
4. Age-Related Changes: As men age, skin elasticity and thickness can decrease, leading to thinning foreskin.
5. Poor Hygiene: Inadequate cleaning can lead to the buildup of smegma, which can irritate the skin and lead to inflammation and thinning.
Treatment Options
1. Consult a Healthcare Provider: The first step is to consult a urologist or dermatologist who specializes in genital skin conditions. They can provide a thorough examination and determine the underlying cause of the thinning foreskin.
2. Topical Treatments: Depending on the diagnosis, your doctor may recommend specific topical treatments. If inflammation is present, a mild topical steroid may be prescribed to reduce inflammation without causing further thinning. However, it’s crucial to use these medications under medical supervision to avoid complications.
3. Moisturizers: Using a gentle, fragrance-free moisturizer can help keep the skin hydrated and reduce irritation. This can be particularly helpful if the skin is dry or cracked.
4. Avoid Irritants: Identify and avoid any products that may irritate the skin, including harsh soaps, fragrances, or other topical medications that may exacerbate the condition.
5. Surgical Options: In severe cases where the thinning foreskin leads to recurrent infections, pain, or other complications, surgical options such as circumcision or preputioplasty may be considered. These procedures can help alleviate symptoms and prevent further complications.
6. Lifestyle Modifications: Maintaining good hygiene practices is essential. Regular cleaning of the genital area with mild soap and water can help prevent infections and irritation.
7. Monitoring: Keep an eye on any changes in the condition of your foreskin. If you notice increased pain, bleeding, or other concerning symptoms, seek medical attention promptly.
Conclusion
Thinning foreskin can result from various factors, including chronic inflammation, skin conditions, and overuse of topical steroids. It is essential to consult a healthcare provider to determine the underlying cause and appropriate treatment options. With proper management, most individuals can find relief from symptoms and prevent further complications. Remember, self-diagnosing and treating without professional guidance can lead to worsening of the condition, so seeking expert advice is crucial.
