Interactions between dietary supplements and medications?
Hello, pharmacist.
Recently, a family member has been diagnosed with high blood pressure and has had a stent placed.
They take aspirin and other anticoagulant medications around 10 AM daily.
Due to a lack of knowledge, they have also been taking fish oil (approximately DHA 333 mg + EPA 400 mg) every night at 10 PM.
Today, I found out that aspirin and fish oil should not be taken together.
Can they continue to take both? Is there any danger in doing so, or is the long digestion time enough to consider them not taken simultaneously? Since the follow-up appointment is still a while away, I can't ask the doctor immediately.
I appreciate your patience in reading this.
Thank you.

Dr. Hou Fangmin reply Pharmacology
Hello Helen: 1.
The combination of DHA 333 mg + EPA 400 mg with aspirin increases the risk of bleeding.
Elderly individuals have slower metabolism and poorer liver and kidney function compared to younger people, making them more susceptible to side effects.
2.
The prolonged digestion time means they are not considered to be taken simultaneously? Because the concentration in the blood still exists, and since aspirin is already an anticoagulant, there is no need to add fish oil.
If you have any other discomfort, please consult your family's primary physician.
Wishing you and your family peace, joy, and good health.

When it comes to the interactions between fish oil and blood thinners, such as aspirin, it is essential to understand the implications of combining these substances, especially for individuals with specific health conditions like hypertension and those who have undergone procedures such as stent placement.
Fish oil is rich in omega-3 fatty acids, particularly EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid), which are known for their cardiovascular benefits. They can help lower triglyceride levels, reduce inflammation, and may even have a mild blood-thinning effect. This is where the concern arises when fish oil is taken alongside anticoagulants like aspirin. Both substances can inhibit platelet aggregation, which is the process that leads to blood clot formation. While this can be beneficial for preventing clots, it also raises the risk of excessive bleeding, especially in individuals who are already on blood-thinning medications.
In your case, your family member is taking aspirin daily for blood thinning and blood pressure management. The concern about whether it is safe to continue taking fish oil alongside aspirin is valid. While there is no absolute contraindication for taking fish oil with aspirin, it is crucial to monitor for any signs of increased bleeding, such as unusual bruising, prolonged bleeding from cuts, or blood in urine or stool.
The timing of ingestion can also play a role. If your family member takes fish oil at bedtime and aspirin in the morning, there may be a reduced risk of interaction simply due to the time gap. However, the cumulative effect of both substances on platelet function should still be considered.
It is advisable to consult with a healthcare provider, such as a physician or a pharmacist, who can provide personalized advice based on your family member's overall health status, the specific dosages of the medications, and any other medications or supplements they may be taking. They can also help assess the risk of bleeding and determine whether adjustments to the regimen are necessary.
In the meantime, if your family member continues to take fish oil, it would be prudent to monitor for any signs of bleeding and to maintain open communication with healthcare providers about any changes in health status or medication regimen. Additionally, lifestyle modifications such as a balanced diet rich in fruits, vegetables, whole grains, and lean proteins can further support cardiovascular health and may help manage blood pressure levels.
In summary, while fish oil can offer cardiovascular benefits, its use alongside blood thinners like aspirin should be approached with caution. Regular follow-ups with healthcare providers are essential to ensure safety and efficacy in managing health conditions.
