Right upper quadrant pain?

Hello, doctor.
For about 1 to 2 years, I have experienced occasional dull or sharp pain in my upper right abdomen.
Recently, it has been happening more frequently, with a few episodes each week (the duration of the pain varies, but sometimes it lasts quite a while).
I had an endoscopy last year that was normal, and an ultrasound two years ago that was also normal.
I would like to ask what this issue might be.
Could it possibly be gallstones?

Huang, 20~29 year old female. Ask Date: 2021/12/18

Dr. Chen Shidian reply Gastroenterology and Hepatology

It may just be neuromuscular pain; you can try applying a warm compress.

Right upper abdominal pain can be a complex issue, as it may arise from various underlying conditions. Given your history of intermittent pain over the past 1 to 2 years, it is essential to consider several potential causes, especially since you have already undergone a gastroscopy and ultrasound, both of which returned normal results.

One of the most common causes of right upper abdominal pain is related to the gallbladder, particularly gallstones. Gallstones can form when there is an imbalance in the substances that make up bile, leading to the formation of solid particles. If a gallstone obstructs the bile duct, it can cause intense pain, often referred to as biliary colic. This pain typically occurs after eating, especially after a fatty meal, and can be accompanied by nausea or vomiting. However, it is important to note that not all gallstones cause symptoms, and some individuals may have them without experiencing pain.

Another potential cause of right upper abdominal pain is liver-related issues. Conditions such as fatty liver disease, hepatitis, or liver tumors can lead to discomfort in this area. Given your history of chronic hepatitis B, it is crucial to monitor liver function regularly, as chronic liver disease can sometimes present with vague abdominal discomfort.

Additionally, gastrointestinal issues such as gastritis, peptic ulcers, or even functional gastrointestinal disorders like irritable bowel syndrome (IBS) can manifest as upper abdominal pain. While you mentioned that previous endoscopic evaluations were normal, it is still possible for functional disorders to cause symptoms without any visible abnormalities on imaging or endoscopy.

Musculoskeletal pain is another consideration, particularly if the pain is exacerbated by movement or certain positions. Strain or injury to the muscles in the abdominal wall or the diaphragm can lead to localized pain that may mimic visceral pain.

Anxiety and stress can also play a significant role in abdominal pain. Psychological factors can lead to increased muscle tension and gastrointestinal motility changes, which may contribute to discomfort. If you have a history of anxiety, it might be worthwhile to explore this aspect further, as managing stress could potentially alleviate some of your symptoms.

Given that your symptoms have persisted and even increased in frequency, it is advisable to consult with a healthcare provider for a thorough evaluation. This may include additional imaging studies, such as a CT scan or MRI, to assess for any abnormalities that may not have been detected in previous tests. Blood tests to evaluate liver function and check for signs of inflammation or infection may also be warranted.

In summary, right upper abdominal pain can stem from various causes, including gallstones, liver conditions, gastrointestinal disorders, musculoskeletal issues, and even psychological factors. A comprehensive evaluation by a healthcare professional is essential to determine the underlying cause and develop an appropriate management plan. If gallstones are suspected, further imaging studies may be necessary to confirm the diagnosis and assess the need for potential treatment options, such as surgery.
