Urticaria after ejaculation
Hello Dr.
Lee, I have two long-standing issues I would like to consult you about.
1.
I am currently taking Sertraline to manage my emotions because I suffer from severe cholinergic urticaria.
Whenever I experience emotional stress or excitement, it triggers hives.
After ejaculation, the emotional arousal causes small red spots of hives on my lower body, particularly on my feet, which subside after resting.
I haven't mentioned this to my psychiatrist because it's a bit difficult to discuss.
This condition does not affect my daily life, but I am concerned and would like to know if it is serious.
2.
After retracting my foreskin, I noticed a small white spot near the frenulum that resembles a pimple, which can expel a white substance similar to that of a pimple.
It appears to be located at the junction between the glans and the skin, and I cannot determine if it is on the penis or the skin itself.
There is no pain, itching, or any other symptoms associated with it.
I remember this small white spot has been present for many years.
Is this something serious that needs to be addressed? It does not look like a pearly penile papule or a sebaceous cyst.
It truly resembles a pimple, but I cannot find any information on this condition, so I would like to consult you.

Dr. Li Xinyi reply Urology
Emotional stress and excitement can indeed trigger hives.
While it may seem trivial, it is not insignificant.
Hives indicate an autoimmune response, which is essentially an allergic reaction.
The severity of allergies cannot be predicted.
It may be advisable to take antihistamines in advance to prevent allergic reactions.
As for the small white spots you mentioned, they are quite common and usually not a cause for concern.
It is recommended not to squeeze them, as this could lead to infection.
If you are worried, it is best to consult a urologist for an examination and discuss it with your physician.

When it comes to understanding post-ejaculation hives and skin concerns in men, it’s essential to address both the physical and psychological aspects of your symptoms.
1. Cholinergic Urticaria and Emotional Triggers: Cholinergic urticaria is a condition characterized by hives that occur in response to an increase in body temperature, often triggered by exercise, heat, or emotional stress. Your experience of developing hives after ejaculation may be linked to the emotional excitement and physiological changes that occur during sexual arousal and orgasm. The release of certain hormones and neurotransmitters during these moments can lead to increased body temperature and sweating, which may trigger your cholinergic urticaria.
While this condition can be uncomfortable, it is generally not dangerous. However, if it significantly impacts your quality of life or causes anxiety, it would be beneficial to discuss this with your psychiatrist or a dermatologist. They may suggest antihistamines or other treatments to help manage your symptoms. It’s important to communicate openly with your healthcare provider about all your symptoms, even those that feel embarrassing, as they can provide the best care when they have a complete picture of your health.
2. Concern About the White Bump: The small white bump you describe near the frenulum (the band of tissue on the underside of the penis) that resembles a pimple and produces a white secretion could be a sebaceous cyst or a type of pearly penile papule. Sebaceous cysts are common and typically harmless, arising from blocked sebaceous glands. Pearly penile papules are also benign and are considered a normal anatomical variant.
Given that the bump has been present for many years without any associated pain, itching, or changes in appearance, it is likely not a cause for concern. However, if you notice any changes in size, color, or if it becomes painful, it would be wise to consult a healthcare professional for an evaluation. They can provide a definitive diagnosis and discuss potential treatment options if necessary.
In summary, while your symptoms may be concerning, they are often manageable with the right approach. It’s crucial to maintain open communication with your healthcare providers about all aspects of your health, including those that may feel uncomfortable to discuss. This will ensure you receive comprehensive care tailored to your needs. If you have further questions or if your symptoms change, don’t hesitate to seek medical advice.
