Right scapular pain and the menstrual cycle?

Hello, I experience severe pain in my right scapula one day before my menstrual period starts.
The pain does not improve with massage or physical therapy.
The pain is located within the scapular region, and when it becomes sharp, it radiates throughout the entire right side of my back.
Pain relievers are not very effective, but steroid pain relief patches provide some improvement.
This pain occurs with remarkable consistency every month, always before my period, and subsides after menstruation begins, only to return the following month.
There is limited information available online, and I would like to know which specialist I should consult.
Thank you.

Hello! It sounds like you're experiencing a recurring pain in your right shoulder blade that coincides with your menstrual cycle. This type of pain can be quite distressing, especially when it disrupts your daily activities and doesn't respond well to typical treatments like physical therapy or pain medications.

The pain you're describing, which intensifies just before your menstrual period and improves afterward, may be related to hormonal changes that occur during your cycle. Hormones such as estrogen and progesterone fluctuate significantly before and during menstruation, and these changes can affect muscle tension, inflammation, and pain perception. In some women, these hormonal fluctuations can lead to increased muscle tension or spasms, particularly in the neck, shoulders, and upper back areas, which could explain the pain you're experiencing in your shoulder blade.

Additionally, the location of your pain—specifically in the area of the shoulder blade—could suggest that it is related to the muscles and soft tissues in that region. Conditions like myofascial pain syndrome, where trigger points in the muscles can cause referred pain, might also be a factor. The fact that you find relief with steroid patches suggests that inflammation could be playing a role in your symptoms.

Given the cyclical nature of your pain, it would be advisable to consult with a healthcare provider who specializes in women's health, such as a gynecologist, as well as a pain management specialist or a physical therapist who has experience with musculoskeletal pain. They can help determine if your shoulder pain is indeed linked to your menstrual cycle or if there are other underlying issues that need to be addressed.

In the meantime, here are a few strategies you might consider to help manage your pain:
1. Heat Therapy: Applying heat to the affected area can help relax tight muscles and alleviate pain. You might try using a warm compress or heating pad on your shoulder blade.

2. Gentle Stretching: Incorporating gentle stretching exercises for your neck and shoulders can help relieve tension. Focus on stretches that target the upper back and shoulder areas.

3. Posture Awareness: Pay attention to your posture, especially if you spend long periods sitting or working at a desk. Poor posture can contribute to muscle tension and pain.

4. Stress Management: Since stress can exacerbate muscle tension, consider incorporating relaxation techniques such as deep breathing, yoga, or meditation into your routine.

5. Diet and Hydration: Maintaining a balanced diet and staying hydrated can also help manage inflammation in the body.

6. Consultation with Specialists: As mentioned earlier, seeing a gynecologist for a thorough evaluation of your menstrual cycle and a pain management specialist for your shoulder pain could provide you with more targeted treatment options.

In summary, your shoulder blade pain appears to be linked to your menstrual cycle, potentially due to hormonal changes affecting muscle tension and inflammation. Seeking advice from a healthcare provider who specializes in women's health and pain management will be beneficial in addressing your symptoms effectively. Remember to keep track of your pain patterns and any other symptoms you may experience, as this information can be valuable during your consultations. Wishing you relief and better health!
