The issues related to Metotrexate and oral isotretinoin?

I have previously had issues with acne and have been taking oral isotretinoin.
Currently, I am taking one 20mg capsule of oral isotretinoin per week.

When considering the interaction between Mycoterk (which contains terbinafine, an antifungal medication) and oral A-retinoids (like isotretinoin, commonly used for severe acne), it's essential to understand how these medications work and their potential effects on the body.

Oral A-retinoids, such as isotretinoin, are powerful medications that significantly reduce sebum production, promote skin cell turnover, and have anti-inflammatory properties. They are often prescribed for severe acne that has not responded to other treatments. However, isotretinoin can have several side effects, including dry skin, mucous membrane dryness, and potential liver toxicity, which necessitates regular monitoring of liver function and lipid levels during treatment.

On the other hand, Mycoterk (terbinafine) is primarily used to treat fungal infections, such as onychomycosis (fungal nail infections) and tinea (ringworm). It works by inhibiting the synthesis of ergosterol, an essential component of fungal cell membranes. While terbinafine is generally well-tolerated, it can also have side effects, including liver enzyme elevation, gastrointestinal disturbances, and skin reactions.

The primary concern when taking these two medications together is the potential for increased liver toxicity. Both isotretinoin and terbinafine can affect liver function, and combining them may amplify this risk. Therefore, it is crucial to monitor liver function tests (LFTs) regularly while on these medications, especially if they are taken concurrently.

In terms of acne treatment efficacy, taking terbinafine should not directly interfere with the effectiveness of isotretinoin in treating acne. However, the side effects associated with both medications could complicate the treatment process. For instance, if you experience increased liver enzyme levels or other adverse effects, your healthcare provider may need to adjust the dosages or consider alternative treatments.

If you are currently taking isotretinoin and considering starting Mycoterk, it is vital to consult your healthcare provider or dermatologist. They can evaluate your specific situation, including your overall health, the severity of your acne, and the fungal infection's extent. They may recommend close monitoring of your liver function and possibly adjusting the treatment plan based on your response to the medications.

In summary, while taking Mycoterk with oral A-retinoids like isotretinoin may not directly affect the acne treatment's effectiveness, the potential for increased liver toxicity and other side effects necessitates careful monitoring and consultation with your healthcare provider. Always prioritize open communication with your healthcare team about any new medications you plan to start while on existing treatments.
