Possible Side Effects of BNT Vaccine: Heart Issues in Young Adults - Cardiology

Suspected side effects of the BNT vaccine?

Hello, Doctor.
I'm inquiring on behalf of my boyfriend (28 years old).
He received his third dose of the BNT vaccine on February 10 (his first two doses were AstraZeneca).
Since last week, he has been experiencing shortness of breath and an increased heart rate.
He visited an ENT clinic, and after auscultation, the doctor noted that his heart was beating strongly and prescribed medication for post-nasal drip.
I would like to ask Dr.
Wu if it is possible that these symptoms could be side effects of the vaccine, or early symptoms of myocarditis? However, it has been over a month since he received the vaccine, so could it just be symptoms of a cold? Thank you.

Weiwei, 20~29 year old female. Ask Date: 2022/03/28

Dr. Wu Xueming reply Cardiology

It has been over a month since the vaccination, and the likelihood of vaccine-related side effects is low.
However, it is still recommended to visit a cardiology clinic for an evaluation.

The concerns regarding potential side effects of the BNT (Pfizer-BioNTech) vaccine, particularly in relation to heart issues such as myocarditis, are valid and have been the subject of ongoing research and discussion. It is important to understand the context of these concerns, especially for young adults who may be more susceptible to certain vaccine-related side effects.

Myocarditis, which is inflammation of the heart muscle, has been reported as a rare side effect following mRNA COVID-19 vaccinations, including the BNT vaccine. The incidence of myocarditis appears to be higher in younger males, particularly after the second dose of the vaccine. However, it is crucial to note that the overall risk remains low. According to data from health authorities, the risk of developing myocarditis after vaccination is estimated to be around 1 in 10,000 to 1 in 100,000 doses, depending on age and gender.

In your boyfriend's case, he received his third dose of the BNT vaccine on February 10, and he is now experiencing symptoms such as shortness of breath and an increased heart rate. Given that it has been over a month since his vaccination, it is less likely that these symptoms are directly related to the vaccine. Vaccine-related myocarditis typically presents within a few days after vaccination, and symptoms may include chest pain, shortness of breath, or feelings of having a fast-beating, fluttering, or pounding heart.

The symptoms he is experiencing could potentially be attributed to a variety of other causes, including respiratory infections, anxiety, or other underlying health conditions. The fact that he has been evaluated by an ear, nose, and throat specialist who noted a strong heartbeat suggests that there may be other factors at play. It is also possible that he could be experiencing symptoms related to post-viral syndromes if he had a recent respiratory infection.

Given the complexity of the situation, it is advisable for your boyfriend to consult a cardiologist for a thorough evaluation. This may include an electrocardiogram (ECG), echocardiogram, and possibly blood tests to assess cardiac enzymes. These tests can help determine if there is any underlying cardiac issue that needs to be addressed.

In summary, while myocarditis is a potential side effect of the BNT vaccine, the timing of your boyfriend's symptoms—occurring over a month after vaccination—makes it less likely that they are vaccine-related. It is essential to rule out other causes of his symptoms through appropriate medical evaluation. If he continues to experience symptoms or if they worsen, seeking immediate medical attention is crucial to ensure his health and safety.
In addition to medical evaluation, maintaining a healthy lifestyle, including regular exercise, a balanced diet, and stress management, can contribute positively to heart health. If there are concerns about cholesterol levels or other cardiovascular risk factors, these should also be discussed with a healthcare provider to determine the best course of action.
