The impact of COVID-19 vaccines on ankylosing spondylitis?
Hello Doctor, I previously asked you about the worsening of my ankylosing spondylitis condition after receiving the first dose of the BNT vaccine (where my medication became ineffective in controlling inflammation levels and pain) (#177836).
At that time, you mentioned that "the duration of the inflammatory response worsening due to the COVID-19 vaccine varies, but it generally lasts for several months." I would like to ask you two more questions:
1) The COVID-19 vaccine does not necessarily worsen ankylosing spondylitis, but if it does worsen the condition in some individuals, is it due to the antibodies produced by the vaccine? Is the reason for the worsening lasting "several months" related to the decline in antibody levels over that time, leading to a stabilization of the ankylosing spondylitis condition?
2) If it is not related to antibodies, what could be the reason for the worsening of ankylosing spondylitis after receiving the BNT vaccine? I am asking these two questions because before receiving the first dose of the BNT vaccine, I was able to control my condition with NSAIDs, but after the first dose, I had to add Salazopyrin.
I am concerned that after the second dose, I may need to start biologics.
If I can be assured that the impact of the COVID-19 vaccine on ankylosing spondylitis is "mostly" only for several months and not for many years, I would feel more confident about getting the second dose.
Thank you, Doctor.

Dr. Zeng Guosen reply Internal Medicine
Hello, the COVID-19 vaccine does not necessarily worsen ankylosing spondylitis; however, it is true that approximately 5% of patients with autoimmune rheumatic diseases may experience instability in their condition after vaccination.
The reasons for this are unclear, and the degree and duration of the effects can vary.
If there is a suspicion that the BNT vaccine is causing issues, it is recommended to switch to a different type of vaccine.
Thank you for your question.

The impact of COVID-19 vaccines, particularly the BNT vaccine (Pfizer-BioNTech), on individuals with ankylosing spondylitis (AS) is a topic of considerable interest and concern among patients. Your questions touch on important aspects of how vaccines can interact with autoimmune conditions like AS, and I will address them in detail.
1. Vaccine-Induced Inflammation and Antibody Response: It is true that some individuals with autoimmune diseases, including AS, may experience a temporary exacerbation of their condition following vaccination. This can be attributed to the immune response triggered by the vaccine. When you receive a vaccine, your body produces antibodies to fight the virus, and this immune activation can sometimes lead to increased inflammation in individuals with pre-existing inflammatory conditions. The duration of this exacerbation can vary, but it is often temporary, lasting for a few months as the immune response stabilizes and the antibody levels begin to decline.
The immune system's response to the vaccine is complex and can vary significantly from person to person. In some cases, the inflammation may be due to the body’s heightened immune activity rather than the antibodies themselves. After a few months, as the immune response normalizes, many patients find that their symptoms improve. Therefore, while the vaccine can trigger a temporary worsening of symptoms, this does not necessarily indicate a long-term deterioration of the underlying condition.
2. Other Factors Influencing Exacerbation: If the exacerbation of AS symptoms is not solely due to the antibody response, other factors may also play a role. For instance, the stress of vaccination, changes in routine, or even the psychological impact of the pandemic and health concerns can contribute to flare-ups in chronic conditions. Additionally, the inflammatory response to the vaccine may interact with the underlying disease mechanisms of AS, leading to increased symptoms.
It is also important to note that the relationship between vaccination and autoimmune diseases is an area of ongoing research. While some patients report worsening symptoms, many others do not experience any change or may even see an improvement in their condition post-vaccination.
Regarding your concerns about needing to escalate treatment to biologics after the second dose of the vaccine, it is essential to have a thorough discussion with your rheumatologist. They can provide personalized advice based on your medical history, current treatment regimen, and the severity of your AS.
In general, the benefits of vaccination against COVID-19, especially for individuals with autoimmune conditions, often outweigh the potential risks. Vaccination can significantly reduce the risk of severe illness from COVID-19, which is particularly important for those with underlying health issues.
If you are considering the second dose of the BNT vaccine, it may be helpful to monitor your symptoms closely and maintain open communication with your healthcare provider. They can guide you on managing any potential flare-ups and adjusting your treatment plan if necessary.
In conclusion, while some individuals with ankylosing spondylitis may experience a temporary worsening of their condition following COVID-19 vaccination, this is not universally the case, and many patients tolerate the vaccine well. The immune response to the vaccine is typically short-lived, and most patients can expect their symptoms to stabilize over time. Your healthcare provider is your best resource for navigating these concerns and ensuring that you receive the appropriate care and support.
