Postoperative care for anal fistula surgery?

Last July, I underwent anal fistula surgery, and the day after the procedure, the wound suddenly healed, prompting a second surgery.
Until February of this year, I experienced minimal issues aside from some swelling after using the restroom.
However, since mid-February, I've noticed that sometimes there is a discharge resembling pus, along with swelling, and it feels like the wound hasn't fully healed.
I would like to know if this is normal and how to address the swelling and discharge.

Wu, 20~29 year old female. Ask Date: 2022/05/02

Dr. Zhang Shuhao reply Surgery

If there is recurrent purulent discharge and swelling at the original surgical site, it may indicate a recurrence of the fistula or the formation of a new anal abscess.
Chronic fistulas can have multiple branches, and the internal glands and epithelial tissue must be thoroughly excised to allow the wound to heal from the inside out, preventing the outer skin from healing first and creating a cavity.
It is recommended to return for a follow-up visit for local drainage and debridement; if it can be managed in an outpatient setting, a repeat surgery may not be necessary.

Post-surgery discomfort and complications following an anal fistula repair can be concerning, and it's important to understand what is considered normal and what may require further medical attention. After undergoing surgery for an anal fistula, it is not uncommon to experience some discomfort, swelling, and even discharge as part of the healing process. However, persistent or worsening symptoms may indicate complications that need to be addressed.

In your case, experiencing swelling and discharge several months after the surgery could be a sign of incomplete healing or even a recurrence of the fistula. Discharge that resembles pus or has an unpleasant odor may suggest an infection. It's essential to monitor the characteristics of the discharge, including its color, consistency, and any accompanying symptoms such as fever or increased pain. If the discharge is clear and minimal, it may be part of the normal healing process, but if it is significant or accompanied by other symptoms, it warrants a visit to your healthcare provider.

To manage discomfort and swelling, consider the following strategies:
1. Warm Sitz Baths: Taking warm sitz baths several times a day can help soothe the area, reduce swelling, and promote healing. This practice can also help with pain relief and relaxation.

2. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate discomfort. Always follow the recommended dosages and consult your doctor if you have any concerns about medication interactions or side effects.

3. Dietary Adjustments: A high-fiber diet can help prevent constipation, which can exacerbate discomfort during bowel movements. Ensure you are drinking plenty of fluids and consuming fruits, vegetables, and whole grains.

4. Hygiene: Keeping the area clean is crucial. After bowel movements, gently clean the area with mild soap and water, and consider using moist wipes to avoid irritation from toilet paper.

5. Avoid Straining: Straining during bowel movements can increase pressure on the surgical site and lead to discomfort. If you experience constipation, consider using stool softeners or fiber supplements as recommended by your healthcare provider.

6. Follow-Up Appointments: Regular follow-ups with your surgeon or healthcare provider are essential for monitoring your recovery. If you notice any changes in your symptoms, such as increased swelling, pain, or changes in discharge, do not hesitate to contact your doctor for further evaluation.

In summary, while some discomfort and discharge can be normal after anal fistula repair, persistent symptoms should not be ignored. It is crucial to maintain open communication with your healthcare provider to ensure proper healing and to address any complications that may arise. If you have concerns about your symptoms or if they worsen, please seek medical attention promptly. Your health and comfort are paramount, and timely intervention can make a significant difference in your recovery process.
