Should I stop taking my medication after contracting COVID-19?

Hello Doctor, I am currently 19 years old and a patient with ankylosing spondylitis.
I am currently on a stable treatment regimen of one tablet of Wanket and two tablets of Salazopyrin daily.
I would like to ask, if I unfortunately contract COVID-19:
1) If I am asymptomatic, which of my current ankylosing spondylitis medications should I stop taking? How long should I stop taking it?
2) If I have mild symptoms and need to take over-the-counter cold medications, which of my current ankylosing spondylitis medications should I stop taking? How long should I stop taking it?
3) If I have moderate to severe symptoms and am prescribed Remdesivir, which of my current ankylosing spondylitis medications should I stop taking? How long should I stop taking it?
Thank you, Doctor.

Dr. Gao Jiankai reply Internal Medicine

Hello, thank you for your question.
Here are my responses: 1) If you are asymptomatic, is there any medication for stiffness that I should stop taking? How long should I stop? --> No need to stop any medication.
2) If you have mild symptoms and need to take general cold medication, is there any medication for stiffness that I should stop taking? How long should I stop? --> If necessary, the physician may prescribe anti-inflammatory medication, in which case you should stop taking Methotrexate.
Therefore, if you do contract the virus and are hospitalized, please be sure to inform your physician about the medications you are currently taking, and they will take care of it.
3) If you have moderate to severe symptoms and are taking Remdesivir, is there any medication for stiffness that I should stop taking? How long should I stop? --> No need to stop any medication.
This is from the Allergy, Immunology, and Rheumatology Department of Changhua Hospital, Ministry of Health and Welfare, caring for you.

When it comes to managing medications after a COVID-19 infection, especially for individuals with pre-existing conditions like ankylosing spondylitis, it is crucial to approach the situation with caution and under the guidance of a healthcare professional. Here’s a detailed response to your questions:
1. If Asymptomatic: If you find yourself infected with COVID-19 but are asymptomatic, there is generally no need to stop your medications for ankylosing spondylitis, which in your case includes one tablet of "Wankes" (likely a non-steroidal anti-inflammatory drug) and two tablets of "Salazopyrin" (sulfasalazine). These medications help manage inflammation and pain associated with your condition. It is advisable to continue taking them as prescribed unless your healthcare provider advises otherwise. Stopping these medications could lead to a flare-up of your symptoms. You should continue them until your healthcare provider gives you a specific recommendation based on your overall health status and any changes in your symptoms.

2. If Mild Symptoms: In the case of mild COVID-19 symptoms, you may need to take over-the-counter medications to alleviate symptoms such as fever, cough, or body aches. If your doctor prescribes anti-inflammatory medications, you may need to stop taking "Wankes" during that time, as combining certain anti-inflammatory drugs can increase the risk of side effects. The duration for which you should stop taking "Wankes" would depend on the specific medications prescribed by your healthcare provider. Always inform your doctor about all medications you are currently taking so they can provide tailored advice.

3. If Moderate to Severe Symptoms: If you experience moderate to severe symptoms and are prescribed antiviral medications like Remdesivir, you typically do not need to stop your ankylosing spondylitis medications. However, it is essential to communicate with your healthcare provider about all medications you are taking, as they will monitor your condition closely and make adjustments as necessary. The duration for which you might need to pause any medication would depend on your response to treatment and any side effects you may experience.

In summary, the general guideline is to continue your medications unless instructed otherwise by your healthcare provider. It is essential to maintain open communication with your healthcare team, especially during a COVID-19 infection, as they can provide the most accurate advice based on your specific health needs and the medications you are taking.
Additionally, managing your overall health during and after a COVID-19 infection is crucial. This includes maintaining a balanced diet, staying hydrated, and monitoring any changes in your symptoms. If you experience any new or worsening symptoms, do not hesitate to seek medical attention. Your healthcare provider can help you navigate the complexities of managing your condition alongside a COVID-19 infection, ensuring that you receive the best possible care.
