Testicular issues?

There is a lump on my left and right testicles that hurts when pressed.
This has only occurred in the past few days, and I have also been engaging in excessive masturbation.
I have researched that if the lump is hard, it could be epididymal cancer, but since it is soft, what could it be?

Hong, 10~19 year old female. Ask Date: 2022/05/09

Dr. Li Minghui reply Urology

Hello: It could be related to the blood vessels or the epididymis.
If there is pain, or if there is swelling or hardness, please seek medical evaluation.

When it comes to testicular issues, particularly painful lumps, it’s essential to approach the situation with caution and a clear understanding of potential causes. The presence of a lump in the testicle, especially if it is painful, can be concerning and warrants further investigation.

Firstly, it’s important to note that not all lumps in the testicles are cancerous. In fact, many conditions can lead to the formation of lumps, and they can vary significantly in their nature and seriousness. Here are some common causes of painful lumps in the testicles:
1. Epididymitis: This is an inflammation of the epididymis, the tube that carries sperm from the testicle. It can be caused by infections, including sexually transmitted infections (STIs) or urinary tract infections. Symptoms often include pain, swelling, and tenderness in the affected testicle.

2. Orchitis: This is an inflammation of one or both testicles, often due to viral or bacterial infections. Mumps virus is a well-known cause of orchitis, especially in individuals who have not been vaccinated.

3. Hydrocele: This condition involves the accumulation of fluid around the testicle, leading to swelling. While hydroceles are usually painless, they can sometimes cause discomfort.

4. Varicocele: This is an enlargement of the veins within the scrotum, which can cause a lump-like feeling and may be associated with discomfort or pain, especially after prolonged standing or physical activity.

5. Testicular torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off blood supply to the testicle. It typically presents with sudden, severe pain and requires immediate medical attention.

6. Tumors: While less common, testicular tumors can present as lumps. These can be benign or malignant. If the lump is hard and does not move freely, it is crucial to seek medical evaluation.

Given that you mentioned the lump is soft and painful, it may be more indicative of conditions like epididymitis or orchitis rather than a tumor. However, the fact that you have been experiencing discomfort and have a history of excessive masturbation adds another layer to consider. While masturbation itself is generally not harmful, excessive activity can lead to temporary discomfort or inflammation in the genital area.

It is crucial to consult a healthcare professional for a thorough examination and appropriate diagnostic tests, such as an ultrasound, to determine the exact nature of the lump. They may also check for signs of infection or other underlying conditions that could be contributing to your symptoms.

In the meantime, here are some general recommendations to manage discomfort:
- Rest: Avoid activities that may exacerbate the pain, including heavy lifting or strenuous exercise.

- Ice: Applying a cold pack to the affected area can help reduce swelling and relieve pain.

- Pain Relief: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, can help manage discomfort.

- Support: Wearing supportive underwear may provide comfort and reduce pain during movement.

In conclusion, while the presence of a painful lump in the testicle can be alarming, many potential causes are treatable. It is essential to seek medical advice to ensure proper diagnosis and management. Early intervention can lead to better outcomes, especially in cases where infection or other treatable conditions are involved.
