A soft mass felt below the right rib cage?
Hello Doctor: I am a 21-year-old female.
I can feel a soft lump just below my right rib cage when I gently press on it (it is not visible on the surface).
It feels very soft, like it contains fluid, and it bounces back when I press down and release.
I can feel it, but it is not painful, and I can touch it whether I am standing or sitting, but not when lying down.
There is also a small one on the left side, but it is not as pronounced as the one on the right.
I am not sure when it appeared.
Could this be a lipoma? Or could I be feeling my liver? Or is it part of my rib cage? Recently, I have been experiencing recurrent abdominal bloating and discomfort due to exercise and my menstrual period, accompanied by low-grade fever and nausea.
I am unsure if this is related to the lump, and I apologize for being very anxious about whether there might be a problem...

Hello: Lipomas do not change shape based on body position.
Generally, if they are not growing continuously, you can leave them alone.
However, if they do grow, please consult a general surgery outpatient clinic.

More Info
When it comes to a soft lump under the right rib, especially in a young woman like yourself, there are several potential causes to consider. The lump you describe as soft, water-like, and non-painful could be a variety of things, including a lipoma (a benign fatty tumor), a cyst, or even a lymph node. Given that it is not visible and only palpable when standing or sitting, it is less likely to be a structural issue like a rib or organ itself, but rather something superficial.
1. Lipoma: This is a common benign tumor made up of fat tissue. Lipomas are usually soft, movable, and painless, which aligns with your description. They can occur anywhere in the body, including the abdomen and near the ribs.
2. Cyst: A cyst is a sac-like structure filled with fluid or semi-solid material. These can also be soft and movable. Epidermoid cysts, for example, can develop under the skin and may feel similar to what you are describing.
3. Lymph Nodes: While less common in the rib area, lymph nodes can become enlarged due to infection or other conditions. They are typically firm but can sometimes feel soft.
4. Muscle or Connective Tissue: Sometimes, what you feel could be related to muscle tissue or fascia, especially if you have been active or have experienced abdominal bloating.
Regarding your symptoms of abdominal bloating, low-grade fever, and nausea, these could be related to a gastrointestinal issue rather than the lump itself. Conditions such as gastritis, irritable bowel syndrome (IBS), or even a mild infection could cause these symptoms. The fact that you mentioned these symptoms coinciding with your menstrual cycle could also indicate hormonal changes affecting your gastrointestinal tract.
Recommendations:
1. Medical Evaluation: It is crucial to consult with a healthcare provider for a thorough evaluation. They may perform a physical examination and possibly imaging studies (like an ultrasound) to better understand the nature of the lump and any underlying issues.
2. Monitoring Symptoms: Keep track of any changes in the lump, such as size, shape, or any new symptoms that may arise. If you experience increased pain, fever, or significant changes in your gastrointestinal symptoms, seek medical attention promptly.
3. Lifestyle Considerations: If your bloating is related to diet or stress, consider keeping a food diary to identify potential triggers. Staying hydrated, eating a balanced diet, and managing stress can help alleviate some gastrointestinal discomfort.
4. Avoid Self-Diagnosis: While it’s natural to be concerned and seek answers, self-diagnosing can lead to unnecessary anxiety. A healthcare professional can provide a more accurate diagnosis and appropriate treatment options.
In summary, while the soft lump under your right rib could be benign, it is essential to have it evaluated by a healthcare professional, especially given your accompanying symptoms. Early assessment can help rule out any serious conditions and provide peace of mind.
