Swelling of the foreskin with pain upon pressure?

Hello Doctor, I have a problem with phimosis.
I usually clean myself while showering, but since the day before yesterday, I have noticed a small wound on my foreskin.
When I retract it, it pulls on the wound and is somewhat painful, but I didn’t pay much attention to it.
Yesterday, suddenly my foreskin became swollen and pressing on it is very painful.
Could this be due to a bacterial infection inside, which is causing the pain? The swelling has progressed to the point where I cannot retract the foreskin to expose the glans, and even the lymph nodes in my groin have started to swell.
Is this a serious condition? What kind of treatment would I receive at the hospital? I’m very worried about potential complications.

More Info

Painful swelling of the foreskin, particularly in the context of phimosis (a condition where the foreskin cannot be easily retracted), can be concerning and may indicate an underlying issue such as infection or inflammation. Based on your description, it seems you are experiencing symptoms that could be related to a condition known as balanitis, which is inflammation of the glans (the head of the penis) and can occur when the foreskin is not adequately cleaned or if there is a bacterial or fungal infection.

Causes of Painful Swelling of the Foreskin
1. Infection: Bacterial infections are common, especially if there are small cuts or abrasions on the skin. These can lead to localized infections that cause swelling, redness, and pain. Fungal infections, such as candidiasis, can also occur, particularly in warm, moist environments.

2. Inflammation: Conditions like balanitis or posthitis (inflammation of the foreskin) can cause swelling and pain. This inflammation can be triggered by irritants such as soaps, lotions, or even certain fabrics.

3. Phimosis: If the foreskin is too tight, it can lead to painful retraction attempts, which may cause small tears and subsequent swelling.

4. Lymphadenopathy: The swelling of the lymph nodes in the groin area can indicate that the body is responding to an infection. This is a sign that the body is fighting off an infection, but it can also indicate a more serious condition that requires medical attention.

Symptoms to Watch For
- Severe Pain: If the pain is severe and persistent, it is essential to seek medical attention.

- Fever: A fever may indicate a systemic infection that requires immediate treatment.

- Discharge: Any unusual discharge from the penis should be evaluated by a healthcare provider.

- Difficulty Urinating: If you experience difficulty urinating or any changes in urinary habits, this is a sign to seek medical help.

Treatment Options
1. Antibiotics: If a bacterial infection is confirmed, antibiotics may be prescribed. It is crucial to complete the entire course of antibiotics as directed.

2. Topical Treatments: Antifungal or antibacterial creams may be applied to the affected area to reduce inflammation and treat the infection.

3. Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and reduce inflammation.

4. Hygiene Practices: Maintaining proper hygiene is essential. Gently cleaning the area with mild soap and water can help prevent further irritation.

5. Circumcision: In cases of recurrent infections or severe phimosis, circumcision may be considered as a long-term solution.

When to Seek Medical Attention
Given your symptoms of painful swelling, inability to retract the foreskin, and swollen lymph nodes, it is advisable to seek medical attention promptly. A healthcare provider will perform a physical examination, possibly order laboratory tests, and determine the best course of action based on the underlying cause of your symptoms.

Conclusion
While it is understandable to be concerned about the severity of your condition, many cases of balanitis or related infections can be effectively treated with appropriate medical care. Early intervention is key to preventing complications and ensuring a full recovery. Do not hesitate to visit a healthcare provider, as they can provide the necessary treatment and guidance tailored to your specific situation.
