Postoperative descent and fecal discharge?

Hello Doctor, I am the one who asked question #184914 yesterday.
I had a natural delivery and underwent surgery for hemorrhoids.
I consulted an obstetrician to inquire if it was related to childbirth, and the obstetrician said I only had a second-degree tear and did not believe there was an issue, thinking it was caused by the hemorrhoid surgery.
I have been experiencing fecal leakage and would like to ask: 1.
If the anal sphincter was possibly injured during childbirth, can it heal over time? Are there any treatment options? 2.
Currently, I also feel a sense of heaviness and pressure in the anal area.
Will this improve over time? 3.
Would you recommend that I see a colorectal specialist or return to the obstetrician for further examination? I have many questions, and I appreciate your time in responding.

Niming, 30~39 year old female. Ask Date: 2022/08/04

Dr. Zhang Shuhao reply Surgery

1.
If the anal sphincter is injured during childbirth, performing Kegel exercises can help strengthen the pelvic floor muscles, and one may also consider undergoing sphincter repair surgery.
2.
Abnormal sensations after anal surgery may improve over time.
3.
It is advisable to monitor the situation for a period of time, and if it causes significant inconvenience in daily life, one should consult a colorectal specialist.

After undergoing hemorrhoid surgery, it is not uncommon for patients to experience a variety of symptoms, including discomfort, changes in bowel habits, and sensations of pressure or heaviness in the anal area. Your concerns about potential complications related to childbirth and the surgery itself are valid and warrant a thorough discussion.

1. Recovery of the Anal Sphincter: If there was any trauma to the anal sphincter during childbirth, it is possible that this could contribute to your current symptoms. The anal sphincter can heal over time, but the extent of recovery often depends on the severity of the injury. In cases of second-degree tears, where the muscle is involved but not completely severed, many women do experience improvement in function and sensation over time. However, if you are experiencing persistent symptoms such as fecal incontinence or a feeling of incomplete evacuation, it is essential to consult with a specialist. Pelvic floor therapy may also be beneficial, as it can help strengthen the muscles around the pelvic area and improve control.

2. Sensation of Heaviness or Pressure: The sensation of heaviness or a feeling of "bearing down" in the anal region can be a common post-operative symptom. This may be due to swelling, inflammation, or the healing process itself. Over time, as the surgical site heals and inflammation decreases, these sensations often improve. However, if the feeling persists or worsens, it could indicate complications such as infection or improper healing, which would require further evaluation.

3. Choosing the Right Specialist: Given your symptoms, it would be advisable to see a colorectal surgeon or a specialist in gastroenterology rather than returning to obstetrics. Colorectal surgeons are specifically trained to handle issues related to the rectum and anus, including complications from hemorrhoid surgery and any potential issues stemming from childbirth. They can perform a thorough examination, possibly including imaging studies or endoscopy, to assess the condition of your anal sphincter and surrounding tissues.

In addition to these points, it is crucial to manage your bowel habits carefully post-surgery. Straining during bowel movements can exacerbate symptoms and hinder healing. A high-fiber diet, adequate hydration, and possibly stool softeners can help maintain regular bowel movements without straining.
If you experience any signs of infection, such as increased pain, fever, or unusual discharge, seek medical attention promptly. Your healthcare provider may also recommend follow-up appointments to monitor your recovery and address any ongoing issues.

In summary, while some discomfort and changes in sensation are normal after hemorrhoid surgery, persistent or worsening symptoms should be evaluated by a specialist. Early intervention can lead to better outcomes and help you regain comfort and normal function.
