Itching of the glans?
Hello doctor, I have a condition related to phimosis, but I can retract the foreskin freely.
Recently, I tried to retract the foreskin but encountered an issue; after retracting, there was some itching near the frenulum, but when I pushed it back into the phimosis position, the itching went away.

Hello: If there is no redness, keep the area clean and dry, and do not apply too much pressure when doing it yourself.
If there is redness, seek medical attention, and it should be able to recover.
Wishing you good health.

More Info
Itchy glans, particularly in the context of phimosis, can be a source of discomfort and concern for many individuals. Phimosis is a condition where the foreskin cannot be easily retracted over the glans (the head of the penis). This condition can lead to various symptoms, including itching, irritation, and sometimes pain, particularly when attempting to retract the foreskin.
Causes of Itchy Glans in Phimosis
1. Poor Hygiene: One of the most common causes of itching in the glans is inadequate hygiene. The area under the foreskin can accumulate smegma, a combination of dead skin cells, oils, and moisture. This buildup can lead to irritation and itching.
2. Infections: Fungal infections, such as candidiasis (thrush), or bacterial infections can also cause itching. These infections can thrive in the warm, moist environment under the foreskin, especially if hygiene is not maintained.
3. Inflammation: Conditions like balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin) can lead to symptoms of itching and discomfort. These conditions can be caused by infections, irritants, or allergic reactions.
4. Skin Conditions: Dermatitis or other skin conditions such as psoriasis or eczema can also affect the genital area, leading to itching and irritation.
5. Phimosis Itself: The tightness of the foreskin in phimosis can cause friction and irritation, especially during sexual activity or even during normal movement, leading to itching.
Solutions and Management
1. Improved Hygiene: The first step in managing itching is to ensure proper hygiene. Gently retracting the foreskin (if possible) and cleaning the area with mild soap and water can help reduce smegma buildup and prevent infections.
2. Topical Treatments: Over-the-counter antifungal creams or hydrocortisone creams can be applied to alleviate itching and inflammation. However, it's essential to consult a healthcare provider before using any topical treatments, especially in sensitive areas.
3. Avoid Irritants: Avoiding scented soaps, lotions, or other products that may irritate the genital area is crucial. Opt for hypoallergenic products designed for sensitive skin.
4. Medical Consultation: If the itching persists or is accompanied by other symptoms such as redness, swelling, or discharge, it is essential to consult a healthcare provider. They may perform a physical examination and possibly recommend a biopsy or other tests to rule out infections or other underlying conditions.
5. Consider Circumcision: In cases where phimosis is severe and recurrent infections or significant discomfort occur, circumcision may be considered. This surgical procedure removes the foreskin and can alleviate the symptoms associated with phimosis.
6. Regular Follow-ups: If you have been diagnosed with phimosis, regular follow-ups with a healthcare provider can help monitor the condition and manage any complications that may arise.
Conclusion
Itchy glans in the context of phimosis can be attributed to various factors, including hygiene, infections, and inflammation. Addressing these issues through improved hygiene practices, topical treatments, and medical consultation can significantly alleviate symptoms. If conservative measures do not provide relief, discussing surgical options with a healthcare provider may be necessary. Always prioritize open communication with your healthcare provider to ensure the best management of your symptoms and overall health.
