The efficacy of bone formation with Forteo (teriparatide)?

I underwent lumbar fusion surgery (with screws and Romeo) on July 11 due to rheumatoid arthritis, after a previous surgery in June resulted in bone collapse.
An X-ray on August 19 showed that the surgery was successful, but there is still a slight collapse.
I have the following questions:
1.
Is it sufficient to receive Boniva (ibandronate) in August? Should I also receive Forteo (teriparatide) to prevent further bone collapse?
2.
Are the other medications only effective in inhibiting bone loss, while Forteo is the only one that can actually increase bone formation?
3.
Forteo costs between $14,000 and $18,000 per injection, and I need a total of 18 injections, which I have to administer at home (this is quite challenging).
Does this mean the total cost for the entire treatment is $14,000 multiplied by 18 injections? That would be very expensive; what is the total cost of the treatment?
4.
My lumbar bone density is -2.4, and I experienced a collapse after the lumbar surgery.
How much help can Forteo provide in this situation?
5.
What is the clinical effectiveness of Forteo in patients with osteoporosis?

Forteo (teriparatide) is a synthetic form of parathyroid hormone that has been shown to be effective in treating osteoporosis, particularly in patients who are at high risk for fractures. It works by stimulating bone formation, which is a significant advantage over other osteoporosis treatments that primarily focus on inhibiting bone resorption. Given your situation, where you have experienced vertebral collapse and are concerned about bone density, understanding the role of Forteo in your treatment plan is crucial.

1. Combination with Other Treatments: You mentioned using another medication, likely a bisphosphonate (referred to as 保骼麗), which is effective in reducing bone loss but does not promote new bone formation. Forteo is indeed unique in its ability to stimulate osteoblast activity, leading to increased bone density and strength. If you are at high risk for further fractures, combining these treatments may be beneficial, but it is essential to consult your healthcare provider to tailor a plan that suits your specific needs.

2. Efficacy of Forteo: Forteo is particularly recommended for individuals with severe osteoporosis (T-score of -2.5 or lower) or those who have already experienced fractures. Clinical studies have demonstrated that Forteo can significantly increase bone mineral density and reduce the risk of vertebral and non-vertebral fractures. In your case, with a T-score of -2.4 and a history of vertebral collapse, Forteo could potentially help in improving your bone density and reducing the risk of future fractures.

3. Cost Considerations: The cost of Forteo can be a significant concern, as you noted that it ranges from $14,000 to $18,000 per injection, with a typical treatment course requiring multiple injections over 18 months. This can indeed add up to a substantial financial burden. It is advisable to discuss with your healthcare provider about potential financial assistance programs or insurance coverage options that may help mitigate these costs. In some cases, pharmaceutical companies offer patient assistance programs for those who qualify.

4. Impact on Bone Density: After starting Forteo, many patients see improvements in bone density within the first year of treatment. The extent of improvement can vary based on individual factors such as age, overall health, and adherence to the treatment plan. Regular follow-up with your healthcare provider, including bone density scans, will help monitor your progress and adjust treatment as necessary.

5. Clinical Outcomes: In clinical practice, many patients treated with Forteo have reported positive outcomes, including increased bone density and a reduction in fracture rates. However, individual results can vary, and it is essential to maintain a comprehensive approach to osteoporosis management, including adequate calcium and vitamin D intake, weight-bearing exercises, and lifestyle modifications.

In summary, Forteo represents a valuable option for enhancing bone regeneration and treating osteoporosis, especially in patients with a history of fractures or significant bone loss. It is crucial to have an open dialogue with your healthcare provider to determine the best treatment strategy tailored to your specific condition, including the potential benefits and costs associated with Forteo.
