Bruising after steroid injection for keloids?

Hello, doctor.
Today, after receiving a steroid injection at the hospital for my keloid, I felt a throbbing pain when I got home.
A few hours later, I removed the bandage and noticed a small amount of blood (about the amount typically seen after an injection).
Now, after 10 hours, the throbbing pain has decreased, but there is some bruising near the keloid as shown in the attached image.
The medication used was a triamcinolone injection of 40 mg.
I would like to know if this is related to the side effects of the steroid itself or if it is simply a bruise caused by the injection.
Thank you!

Bleeding is a potential complication associated with injections, and it typically resolves within one to two weeks.

Bruising after a steroid injection, such as triamcinolone, can be concerning for patients, especially when it occurs in sensitive areas like keloids. Understanding the causes and implications of this bruising is essential for managing expectations and ensuring proper care.

Firstly, it’s important to recognize that bruising can occur for a variety of reasons. When a needle is inserted into the skin for an injection, it can damage small blood vessels, leading to localized bleeding under the skin, which manifests as a bruise. This is a common occurrence with any injection, not just steroid injections. Factors such as the technique used by the healthcare provider, the size of the needle, and the depth of the injection can all influence the likelihood of bruising.

In the case of steroid injections, particularly with triamcinolone, there are additional considerations. Steroids can affect the skin and underlying tissues, potentially making them more susceptible to bruising. Corticosteroids like triamcinolone can lead to thinning of the skin (atrophy) and weakening of the blood vessels over time, especially with repeated use. However, this is more of a concern with long-term use rather than a single injection.

The pain you experienced after the injection, which you described as "抽痛" (drawing pain), is not uncommon. This can be due to the injection itself, the local tissue reaction to the steroid, or even the keloid tissue's response. Keloids are abnormal growths of scar tissue that can be sensitive and may react differently to injections compared to normal skin.

After a steroid injection, it is also essential to monitor the injection site for any signs of infection, such as increased redness, warmth, swelling, or discharge. While bruising is typically harmless and resolves on its own, any signs of infection would require prompt medical attention.

In summary, the bruising you observed is likely a result of the injection itself rather than a direct side effect of the steroid medication. However, if you notice any worsening of symptoms, or if the bruising does not improve over the next few days, it would be prudent to follow up with your healthcare provider. They can assess the injection site and determine if any further intervention is necessary.

In terms of managing bruising, applying a cold compress to the area can help reduce swelling and discomfort. Over-the-counter pain relievers, such as acetaminophen or ibuprofen, can also be used to alleviate pain, but be cautious with NSAIDs if you have any contraindications.

Lastly, if you have concerns about the frequency of steroid injections or the management of your keloids, discussing alternative treatments with your healthcare provider may be beneficial. Options such as silicone gel sheets, laser therapy, or other intralesional treatments could be considered depending on your specific situation. Always ensure that you have open communication with your healthcare provider regarding any concerns or side effects you experience during treatment.
