Chronic prostatitis issues?
Hello Doctor, I have been suffering from chronic prostatitis for a year.
During this time, my semen culture has shown bacteria at times and at other times it has not.
The first culture revealed Enterococcus faecalis, and I was prescribed oral Norfloxacin, which was ineffective.
However, the administration of Suanbi via intravenous injection was effective, but the symptoms recurred the next day.
I subsequently had two more sterile tests, but the symptoms did not resolve, including perineal, anal, and genital pain, as well as frequent urination.
The third semen culture showed Klebsiella pneumoniae, and the doctor advised a three-month course of oral Ciprofloxacin.
After one month of treatment with no improvement, I consulted other doctors who had not heard of a three-month regimen and did not recommend it.
On my fourth visit, I changed doctors again, and the result was sterile.
The new doctor diagnosed me with sterile prostatitis and suggested I undergo shockwave therapy, costing 6,000 NT dollars per session for a total of six sessions.
My questions are:

Is it possible for multiple bacterial strains to exist simultaneously? What is the accuracy of semen cultures, given the varying results? Is there a possibility that bacteria are hiding in the prostate and cannot be detected?

It seems that the treatment for sterile prostatitis does not involve antibiotics.
Besides pain relievers, muscle relaxants, and alpha-blockers, are there any other medications that can treat this condition, focusing on a cure rather than just symptom management?

Can shockwave therapy truly cure prostatitis or pelvic pain syndrome? After all, it is a significant financial investment.
