Low Testosterone Levels and Their Impact on Sexual Health - Urology

Low testosterone levels?

Hello doctor, my health check-up showed no signs of the three highs (hypertension, hyperglycemia, and hyperlipidemia), and all values are normal.
I also exercise regularly.
Recently, I visited a urologist to test my testosterone levels, which came back at 408 ng/dL.
Currently, I am experiencing erectile dysfunction, including difficulties maintaining an erection, easy fatigue, and a decrease in morning erections.
Would you recommend testosterone supplementation in this situation? Is a level of 408 considered normal, or is it low for a 29-year-old? If I were to increase my levels to 700 ng/dL, would that potentially help restore my function? Thank you.

Wu, 20~29 year old female. Ask Date: 2022/09/11

Dr. Fu Shizhang reply Urology

Within the normal range, there is no need for supplementation.
Online consultations cannot replace an in-person examination and physical assessment by a physician.
If there is a medical condition, it is still recommended to seek care at a healthcare facility.

Low testosterone levels can significantly impact sexual health and overall well-being. Testosterone is a crucial hormone in males, influencing not only sexual function but also mood, energy levels, and muscle mass. The normal range for testosterone levels can vary, but generally, levels between 300 to 1,000 ng/dL are considered normal for adult males. At 29 years old, a testosterone level of 408 ng/dL is on the lower end of the spectrum, especially considering that testosterone levels typically peak in early adulthood and gradually decline with age.

Your symptoms of erectile dysfunction, decreased morning erections, and fatigue are indeed concerning and could be linked to your testosterone levels. While 408 ng/dL is not necessarily classified as low, it is important to consider the context of your symptoms. The presence of sexual dysfunction, particularly in someone your age, warrants further evaluation.
Before considering testosterone replacement therapy (TRT), it is essential to conduct a comprehensive assessment. This includes a thorough medical history, physical examination, and possibly additional blood tests to rule out other causes of your symptoms, such as thyroid dysfunction, diabetes, or other hormonal imbalances. It is also important to evaluate lifestyle factors, including diet, exercise, sleep quality, and stress levels, as these can all influence testosterone levels and sexual health.

If testosterone replacement therapy is deemed appropriate, it can potentially help restore your testosterone levels to a more optimal range, which may alleviate your symptoms. However, it is crucial to approach TRT cautiously, as it can have side effects and may not be suitable for everyone. For instance, TRT can lead to increased red blood cell counts, sleep apnea, and may affect prostate health. Regular monitoring by a healthcare provider is essential during treatment.

In terms of your specific question about whether increasing your testosterone level to around 700 ng/dL could restore your sexual function, many patients do report improvements in libido, energy levels, and erectile function with appropriate testosterone therapy. However, individual responses can vary widely, and not all symptoms may resolve with treatment.

In conclusion, while your testosterone level of 408 ng/dL is not definitively low, the symptoms you are experiencing suggest that further evaluation is warranted. Consulting with a healthcare provider who specializes in hormonal health can help determine the best course of action for your situation. They can guide you through the decision-making process regarding testosterone supplementation and monitor your health throughout treatment. Remember, addressing lifestyle factors and underlying health conditions is equally important in managing sexual health and overall well-being.
